Recalls and investigations

  1. Air bags, frontal, passenger side, inflator moduleOct 17, 2019

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2010-2016 4Runner, 2003-2006 Tundra, 2003-2013 Corolla, 2009-2010 Corolla Matrix, 2004-2005 RAV4, 2002-2007 Sequoia, 2011-2013 Sienna, 2008-2012 Scion xB, 2008-2009 Lexus IS-F, 2007-2012 Yaris and Lexus ES350, 2010-2017 Lexus GX460, 2002-2010 Lexus SC430, 2006-2012 Lexus IS250 and IS350 and 2010-2015 Lexus IS250C and IS350C vehicles. These vehicles had their driver or passenger frontal air bag inflators previously replaced under a prior recall using inflators of the same design. The inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.

An inflator explosion may result in sharp metal fragments striking the driver or other occupants resulting in serious injury or death.

Toyota will notify owners. Depending on the vehicle model, dealers will replace the front driver inflator, front passenger airbag inflator, the airbag assembly, or the front passenger airbag sub-assembly and inflator. The recall began November 20, 2019. Owners may contact Toyota customer service at 1-888-270-9371 or Lexus customer service at 1-800-255-3987. Toyota's numbers for this recall are J0A, J0B, and J0C. Lexus' numbers for this recall are JLI, JLJ, and JLK.

  2. Air bags, frontal, passenger side, inflator moduleAug 28, 2019

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2003-2008 Corolla, 2005-2008 Corolla Matrix, and Pontiac Vibe vehicles equipped with a front passenger air bag assembly replaced under a prior recall. The air bag may not unfold as designed during inflation under high temperature conditions, possibly resulting in the air bag not inflating properly.

An improperly inflated air bag increases the risk of injury during a crash.

Toyota will notify their owners. Pontiac Vibe owners will be notified by GM. Dealers will replace the front passenger air bag assembly or sub-assembly, free of charge. Toyota owners may contact Toyota customer service at 1-888-270-9371. Toyota's numbers for this recall are K1Q and K0Q. Pontiac owners may contact GM customer service at 1-800-762-2737. GM's number for this recall is N192273760.

  3. Air bags, frontal, passenger side, inflator moduleDec 12, 2018

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2002-2005 Lexus SC and Toyota Sequoia, 2003-2005 Toyota Corolla and Tundra, and 2003-2007 Pontiac Vibe vehicles. These vehicles are equipped with certain air bag inflators assembled as part of the passenger frontal air bag modules used as original equipment or replacement equipment. In the event of a crash necessitating deployment of the passenger frontal air bag, these inflators may explode due to propellant degradation.

An inflator explosion may result in sharp metal fragments striking the driver or other occupants resulting in serious injury or death.

Toyota will notify the Toyota and Lexus owners. General Motors will notify the Pontiac Vibe owners. Depending on the vehicle model, dealers will replace the front passenger air bag inflator with an alternate inflator, or replace the air bag assembly that includes an alternate inflator, free of charge. The recall began January 23, 2019. Owners may contact Toyota customer service at 1-888-270-9371, Lexus customer service at 1-800-255-3987 or Pontiac customer service at 1-800-762-2737. Toyota's numbers for these recalls are J0A (Zone A Toyota), J0B (Zone B Toyota), J0C (Zone C Toyota), JLI (Zone A Lexus), JLJ (Zone B Lexus), and JLK (Zone C Lexus). Note: This recall includes vehicles that have had their air bag previously replaced under an earlier air bag campaign.

  4. Air bagsMay 13, 2015

Toyota Motor Engineering & Manufacturing (Toyota) notified the agency on May 13, 2015, that they are recalling 2005-2007 Lexus SC, Toyota Corolla, Corolla Matrix, Sequoia, and Pontiac Vibe, and 2005-2006 Toyota Tundra vehicles that were originally sold, or ever registered, in Florida, along the Gulf Coast, Puerto Rico, Hawaii, the U.S. Virgin Islands, Guam, Saipan and American Samoa. On June 16, 2015 Toyota expanded the recall to cover an additional 769,055 vehicles, including certain model year 2002-2004 Lexus SC, Toyota Sequoia, 2003-2004 Toyota Corolla Matrix, Toyota Corolla, Pontiac Vibe, and Toyota Tundra vehicles, as well the vehicles covered by the May 13, 2015 notification in the portions of Georgia, Alabama, Louisiana, Mississippi and Texas that were not previously included. The inflators in the front passenger air bags are susceptible to rupture in the event of a crash necessitating the deployment of the front passenger air bag.

In the event of a crash necessitating deployment of the passenger's frontal air bag, the inflator could rupture with metal fragments striking the vehicle occupants potentially resulting in serious injury or death.

2003-2004 Corolla and Corolla Matrix vehicles that are recalled under both 15V-043 and 15V-286 are under a Do Not Drive advisory. Owners of these vehicles are advised not to drive their vehicles until the recall remedy has been completed. Toyota will notify their owners. Pontiac Vibe owners will be notified by GM. Dealers will replace the passenger side front air bag, free of charge. Owners may contact Toyota customer service at 1-800-331-4331. Toyota began the recall on July 13, 2015. Pontiac Vibe owners may contact GM at 1-800-762-2737. GM began the recall on June 29, 2015. NOTE: This recacll supersedes recall 14V-655 in its entirety. For vehicles located in the U.S. states not listed above, please see Toyota recall 15V-285 which covers all other U.S. states.

  5. Air bagsMay 13, 2015

Toyota Motor Engineering & Manufacturing (Toyota) notified the agency on May 13, 2015 that they are recalling certain model year 2004 Sequoia vehicles manufactured August 18, 2003, to August 19, 2004, and 2003-2004 Tundra vehicles manufactured March 18, 2003, to August 25, 2004. On June 16, 2015, Toyota expanded the recall to include an additional 1,270,468 vehicles, including model year 2002-2003 and 2005-2007 Sequoia, 2005-2006 Tundra, 2003-2007 Corolla, 2003-2007 Corolla Matrix, 2003-2007 Pontiac Vibe and 2002-2007 Lexus SC vehicles. This is to address a safety defect in the passenger side frontal air bag inflator which may produce excessive internal pressure causing the inflator to rupture upon deployment of the air bag. This recall addresses both the passenger side frontal air bags that were originally installed in the vehicles, as well as replacement air bags that may have been installed as replacement service parts. A replacement air bag may have been installed, as one example, if a vehicle had been in a crash necessitating the replacement of the passenger side frontal air bag.

In the event of a crash necessitating deployment of the passenger's frontal air bag, the inflator could rupture with metal fragments striking the vehicle occupants potentially resulting in serious injury or death.

2003-2004 Corolla and Corolla Matrix vehicles that are recalled under both 15V-043 and 15V-285 are under a Do Not Drive advisory. Owners of these vehicles are advised not to drive their vehicles until the recall remedy has been completed. Toyota will notify their owners. Pontiac Vibe owners will be notified by GM. Dealers will replace the front passenger air bag. The recall began on July 13, 2015. Toyota owners may contact Toyota customer service at 1-800-331-4331. Pontiac owners may call 1-800-762-2737. NOTE: This recall supersedes recall 14V-312 in its entirety.

  6. Air bags, frontalJan 28, 2015

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2003-2004 Toyota Corolla vehicles manufactured December 28, 2001, to May 2, 2004, 2003-2004 Toyota Corolla Matrix vehicles manufactured January 6, 2002, to April 29, 2004, 2003-2004 Avalon vehicles manufactured June 5, 2002, to December 20, 2004, and 2003-2004 Pontiac Vibe vehicles manufactured January 18, 2002, to April 27, 2004. Due to electrical noise, a component in the air bag control module may fail, causing the front air bags, side air bags, and/or seatbelt pretensioners to deploy inadvertently while the vehicle is being operated.

Inadvertent deployment of the air bags or the seat belt pretensioners increases the risk of an injury or crash.

Toyota will notify their owners. Pontiac Vibe owners will be notified by GM. Dealers will replace the air bag control module, free of charge. The recall began on August 24, 2015. Toyota owners may contact Toyota customer service at 1-800-331-4331. GM issued an interim notification to owners on March 27, 2015. The recall began on August 24, 2015. Pontiac owners may call 1-800-620-7668. Note: This recall supersedes recalls 13V-029 and 14V-147. Vehicles that had a noise filter installed as the remedy for one of those campaigns need to have the air bag control module replaced under this campaign.

  7. Air bagsJan 30, 2013

Toyota is recalling certain model year 2003-2004 Corolla and Corolla Matrix vehicles and 2003-2004 Pontiac Vibe vehicles. The supplemental restraint system (SRS) circuits are susceptible to internal shorting. The electrical short may create an abnormal current flow and increased heat which can damage the circuits. This could result in an inadvertent deployment of the front airbags and/or seat belt pretensioners.

Inadvertent deployment of the airbags or the seat belt pretensioners may increase the risk of injury and the possibility of a vehicle crash.

Toyota will re-notify their owners who have already had their vehicles repaired with a noise filter, but did not receive a new airbag control module, and General Motors will re-notify theirs. Dealers previously installed a supplemental harness containing an electrical filter between the airbag control module and its wire harness, free of charge. Dealers will replace the airbag control module, free of charge. The recall began on March 13, 2013. The manufacturer has not yet provided a schedule for second notices. For more information, Toyota owners may call 1-800-331-4331 and Pontiac owners may call 1-800-620-7668.

Car has over 290,000 miles, sole owner of car. Just learned the odometer will freeze at 299,999 miles based on issue known by Toyota for a few models. Odometer freeze was not disclosed even though Toyota has selectively repaired some cars odometers at no charge to customers or split costs with customers. Odometer freeze makes safe maintenance of car more difficult. Toyota apparently added a 9 year warranty extension because of its knowledge of problems but didn't notify owners. My 2004 vehicle would have been covered through 2013, but I was not given an opportunity to have the problem repaired under warranty. Toyota apparently knew this problem affected Corolla (up through 2008), Matrix & Vibe (1st generation) and 1st generation Prius cars. My car manual says that the odometer "shows the total distance the vehicle has been driven." My ability to continue safely using my car will be compromised when my odometer reaches 299,999 and Toyota chose not to warn and repair this defect which affects their customers, and our ability to safely operate our cars on the road. If I had known about the problem sooner I would have addressed it sooner. Toyota knew but chose to only address the issue on a case by case basis even though the cars were apparently designed with this defect which was hidden.

I received a "do not drive" recall for my vehicle in the mail because of defective air bags. The recall letter claimed that there have been twenty-eight confirmed deaths in relation to this recall. I entered Priority Toyota of Chesapeake, VA to address the issue. That was on Monday, April 6th, 2026. Even though there was a "do not drive" order on the recall, Priority Toyota of Chesapeake refused to give me a loaner vehicle. It is now Monday, April 13th, and Priority Toyota of Chesapeake has claimed that they are still waiting on an email to begin repairing the vehicle, which I need for transport. This is a formal complaint that Priority Toyota of Chesapeake, VA has failed to repair the recalls to the vehicle in a timely manner.

  4. Vehicle speed control, Other or unspecified, Service brakes, Feb 20, 2022

My daughter was driving as usual when she went to apply the brakes about 5 car lengths back and the vehicle accelerated uncontrollably despite braking. Her foot was not on the accelerator, she does not have floor mats that would cause anything to hit the accelerator. This has happened on 5-6 occasions which almost resulted in an accident. She barely got stopped mere inches away from the car in front of her despite braking earlier than normal because she encountered this once already during her trip. No check engine codes, the brakes are good, the car idles high. That is really it. This is a serious issue that seemingly cannot be diagnosed. After doing research to see what the problem may be, I have seen countless complaints of a similar nature. My daughters life is invaluable. This needs to be addressed.

MY TOYOTA COROLLA LE (CAR) COMPLETELY LOSES POWER AND SHUTS DOWN RANDOMLY. IT HAS HAPPENED ON NUMEROUS OCCASIONS. DURING ONE OCCASION IT SHUT DOWN WHILE ON THE HIGHWAY. THIS INCIDENT ALMOST COST ME MY LIFE AS WELL AS TWO OF MY CHILDREN (WHO ARE ADULTS). IT COMPLETELY SHUT DOWN IN THE MIDDLE OF A LOT OF TRAFFIC CAUSING A SEMI TRUCK TO MISS US BY INCHES. THIS HAS ALSO HAPPENED ON NUMEROUS OCCASIONS ON STREETS AND IN PARKING LOTS. THE CAR IS ALWAYS IN MOTION WHEN THIS OCCURS. I HAVE HAD THE CAR'S ENGINE TESTED (THE BUSINESS THAT TESTED IT IS NOW OUT OF BUSINESS). THE TEST REVEALED THAT IT WAS A COIL PACK THAT WAS CAUSING THE PROBLEM. I HAD THE COIL PACK REPLACED BUT IT DID NOT FIX THE PROBLEM. AT THIS POINT IT IS STILL UNKNOWN WHY MY VEHICLE SHUTS DOWN RANDOMLY. BELOW IT ASKS FOR A DATE WHEN THIS OCCURS. AS STATED ABOVE, IT HAS HAPPENED SO MANY TIMES THAT I CAN'T RECALL THOSE DATES, SO I PUT IN AN APPROXIMATE DATE THAT THE INCIDENT ON THE HIGHWAY OCCURRED. THE MILES ON THE CAR IS WHAT I HAVE ON IT AT THIS TIME (149,542).

"TAKATA RECALL" I KEEP RECEIVING RECALL NOTICES, BUT WHEN I CALL THE DEALERSHIP, THEY ACT LIKE THEY DON'T KNOW WHAT I AM TALKING ABOUT AND WHEN I CALL THE 1800 NUMBER THEY ACT LIKE EVERYTHING IS TAKEN CARE OF. THEN THE DEALERSHIP CALLS AND THEY DON'T KNOW WHAT IS GOING ON AGAIN. I HAVE GIVEN UP ON THE SAFETY RECALL FOR THE TIME BEING. I HOPE THAT AS TIME PASSES THE SYSTEM WILL GET SMOOTHED OUT WHERE THIS CAN ACTUALLY GET FIXED, MAYBE. IT DOES NOT SEEM THAT FIXING THE AIRBAG ISSUES ARE A SERIOUS PRIORITY IN PERSON, ONLY ON PAPER.

WITHOUT ANY INDICATION OF A POTENTIAL PROBLEM, MY CAR LOST COMPLETE POWER WHILE DRIVING IT. IN ADDITION, MY SPEEDOMETER WAS STUCK ON 50 (I JUST GOT OFF THE INTERSTATE & WAS GOING RESIDENTIAL SPEED), MY GAS GAUGE REFLECTED I HAD A FULL TANK (I ACTUALLY HAD A HALF OF A TANK) AND MY TEMP GAUGE SAID MY ENGINE WAS COMPLETELY COOL. I DON'T KNOW WHAT CAUSED THIS TO HAPPEN, BUT I'M LUCKY THAT IT DID NOT HAPPEN TO ME WHILE I WAS ON THE INTERSTATE! LET ME CLARIFY THE FACTS HERE- TOTAL POWER LOSS WHILE DRIVING, STEERING WHEEL LOCKED UP, GAUGES - INCLUDING THE SPEEDOMETER- WHILE AT A COMPLETE STOP WERE STUCK ON INACCURATE READINGS, NO INDICATION OF AN ISSUE PRIOR TO IT STALLING! (PRIOR TO THE CAR DYING ON ME- HEADLIGHTS AND INTERIOR LIGHTS WERE BRIGHT, RADIO WORKED, GAUGES WERE ACCURATE. THERE WAS NO LOUD NOISE WHEN THIS HAPPENED, IT IS NOT THE TIMING CHAIN, THE STARTER AND BATTERY ARE BRAND NEW, THERE WAS NO HESITATION AND NO TROUBLE WHEN STARTING IT UP.) I HAVE NOT BEEN ABLE TO GET THE CAR TO START SINCE. DID NOT RESPOND TO A JUMP. FROM MY UNDERSTANDING, THERE SHOULD HAVE BEEN SOME ORDER IN WHICH POWER IS LOST - EXAMPLE RADIO FIRST- DIMMING LIGHTS, ETC. BEFORE POWER IS LOST TO THE ENTIRE CAR WHILE DRIVING. *TR

I WAS ON THE HIGHWAY 605 FREEWAY IN CALIFORNIA ON DECEMBER 24,2014 AT 10: AM . I WAS DRIVING 60MPH AND SUDDENLY FELT A VERY STRONG SHAKE IN MY CAR THAT CONTINUED FOR ABOUT TWO MINUTES AND THEN I SUDDENLY WENT FROM 60MPH TO 15 MPH . I WAS REALLY SCARED AND CONCERNED ABOUT THIS MATTER. IF I DIDN'T HAVE CONTROL I WOULD HAVE GOTTEN CARS BEHIND ME OR MYSELF IN A MAJOR ACCIDENT DUE TO THIS MALFUNCTION. I WAS VERY SHAKEN UP AND STRESSED. I HAVE NEVER BEEN THIS SCARED IN MY VEHICLE. I DROVE MY CAR IN SEARCH FOR AN OPEN MECHANIC AND FOUND ONE HE DID A FULL INSPECTION WHICH WAS VERY STRESSFUL DUE TO THE FACT THAT IT WAS A CHRISTMAS EVE . I REALLY HAVE BEEN A LOYAL CUSTOMER TO TOYOTA MY WHOLE FAMILY AND I REALLY WOULD LIKE THIS TO GET TAKEN CARE OF. I DON'T SEE THE NEED FOR ME TO SPEND MORE MONEY WHEN ITS A TOYOTA PROBLEM.

STILL UNTIL THIS DAY, THEY SAY THERE IS NOTHING THAT CAN BE DONE. IT HAS BEEN GETTING WORSE AND MORE OF THE TOP CONSOLE IS POPPING UP CAUSING A RATTLING NOISE. I ALSO SMELL THAT BAD EGG/ SULFUR SMELLING ORDER WHEN I USE THE AC OR THE HEAT. I AM SURE THIS CANT BE GOOD FOR ANYONE'S HEALTH. SOMETHING NEEDS TO BE DONE WITH BOTH OF THESE COMPLAINTS. FACTORY RECALL? *JB

I BOUGHT A TOYOTA COROLLA 2004 SEDAN FROM GORDIE BOUCHER NISSAN, 10606 W. ARTHUR AVENUE, WEST ALLIS, WISCONSIN, PHONE NO. 414-543-1234 ON SEPT. 27TH. THE CAR WAS IN VERY GOOD CONDITION, AND BOTH MY HUSBAND, JOY JOSEPH AND I DIDN'T NOTICE THAT THE HEAD RESTS FOR THE BACK SEATS WERE MISSING. WHEN WE FOUND THAT WE BROGHT THIS TO THE DEALER'S ATTENTION. THEY ARE NOT WILLING TO GET US TWO HEAD RESTS FOR THE BACK. MY HUSBAND WENT TO THE TOYOTA DEALER AS WE STILL HAVE THE FULL WARRANTY FROM TOYOTA, AND THEY ARE ALSO NOT WILLING TO PROVIDE US WITH THE HEAD RESTS. I KNOW THAT IN A COLLISSION IT WILL BE DANGEROUS TO HAVE PASSENGERS IN THE BACK SEAT WHERE THEY WILL HAVE SEVERE NECK INJURIES. BEFORE SOMETHING SEVERE HAPPENS, I WOULD LIKE TO HAVE THE HEAD RESTS IN PLACE. PLEASE HELP ME IN GETTING THIS RESOLVED.

THE DRIVERS SIDE SUN VISOR WILL NOT GO DOWN ALL THE WAY BECAUSE THE REAR VIEW MIRROR IS IN THE WAY (IT HITS IT). THIS IS A DRIVING DISTRACTION TRYING TO REPOSITION THE REAR VIEW MIRROR EVERY TIME THE SUN VISOR IS LOWERED AND RAISED.*AK

THE MOLDING PIECE SEPERATING FROM AROUND THE RADIO. MY CAR WAS 3 MONTHS OLD WHEN THE PROBLEM WAS NOTICED. I HAVE HAD THIS PROBLEM WORKED ON TWICE. THE SECOND TRIP IN,THE MOLDING WAS REPLACED WHICH HAS HELPED FOR THE MOST PART. AT FIRST THERE WAS A BIG GAP ON THE SIDES AND ON TOP OF THE RADIO AND MOLDING. IT STILL IS NOT 100% PERFECT AS I WOULD WISH FOR A BRAND NEW CAR BUT IT LOOKS BETTER NOW THAN IT DID IN THE BEGINNING.
