Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2010-2016 4Runner, 2003-2006 Tundra, 2003-2013 Corolla, 2009-2010 Corolla Matrix, 2004-2005 RAV4, 2002-2007 Sequoia, 2011-2013 Sienna, 2008-2012 Scion xB, 2008-2009 Lexus IS-F, 2007-2012 Yaris and Lexus ES350, 2010-2017 Lexus GX460, 2002-2010 Lexus SC430, 2006-2012 Lexus IS250 and IS350 and 2010-2015 Lexus IS250C and IS350C vehicles. These vehicles had their driver or passenger frontal air bag inflators previously replaced under a prior recall using inflators of the same design. The inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.
An inflator explosion may result in sharp metal fragments striking the driver or other occupants resulting in serious injury or death.
Toyota will notify owners. Depending on the vehicle model, dealers will replace the front driver inflator, front passenger airbag inflator, the airbag assembly, or the front passenger airbag sub-assembly and inflator. The air bag may not unfold as designed during inflation under high temperature conditions, possibly resulting in the air bag not inflating properly.
An improperly inflated air bag increases the risk of injury during a crash.
Toyota will notify their owners. Pontiac Vibe owners will be notified by GM. Dealers will replace the front passenger air bag assembly or sub-assembly, free of charge. These vehicles are equipped with certain air bag inflators assembled as part of the passenger frontal air bag modules used as original equipment or replacement equipment. In the event of a crash necessitating deployment of the passenger frontal air bag, these inflators may explode due to propellant degradation.
An inflator explosion may result in sharp metal fragments striking the driver or other occupants resulting in serious injury or death.
Toyota will notify the Toyota and Lexus owners. General Motors will notify the Pontiac Vibe owners. Depending on the vehicle model, dealers will replace the front passenger air bag inflator with an alternate inflator, or replace the air bag assembly that includes an alternate inflator, free of charge. Toyota Motor Engineering & Manufacturing (Toyota) notified the agency on May 13, 2015, that they are recalling 2005-2007 Lexus SC, Toyota Corolla, Corolla Matrix, Sequoia, and Pontiac Vibe, and 2005-2006 Toyota Tundra vehicles that were originally sold, or ever registered, in Florida, along the Gulf Coast, Puerto Rico, Hawaii, the U.S. Virgin Islands, Guam, Saipan and American Samoa. On June 16, 2015 Toyota expanded the recall to cover an additional 769,055 vehicles, including certain model year 2002-2004 Lexus SC, Toyota Sequoia, 2003-2004 Toyota Corolla Matrix, Toyota Corolla, Pontiac Vibe, and Toyota Tundra vehicles, as well the vehicles covered by the May 13, 2015 notification in the portions of Georgia, Alabama, Louisiana, Mississippi and Texas that were not previously included. The inflators in the front passenger air bags are susceptible to rupture in the event of a crash necessitating the deployment of the front passenger air bag.

Toyota Motor Engineering & Manufacturing (Toyota) notified the agency on May 13, 2015 that they are recalling certain model year 2004 Sequoia vehicles manufactured August 18, 2003, to August 19, 2004, and 2003-2004 Tundra vehicles manufactured March 18, 2003, to August 25, 2004. On June 16, 2015, Toyota expanded the recall to include an additional 1,270,468 vehicles, including model year 2002-2003 and 2005-2007 Sequoia, 2005-2006 Tundra, 2003-2007 Corolla, 2003-2007 Corolla Matrix, 2003-2007 Pontiac Vibe and 2002-2007 Lexus SC vehicles. This is to address a safety defect in the passenger side frontal air bag inflator which may produce excessive internal pressure causing the inflator to rupture upon deployment of the air bag. This recall addresses both the passenger side frontal air bags that were originally installed in the vehicles, as well as replacement air bags that may have been installed as replacement service parts. A replacement air bag may have been installed, as one example, if a vehicle had been in a crash necessitating the replacement of the passenger side frontal air bag.

Due to electrical noise, a component in the air bag control module may fail, causing the front air bags, side air bags, and/or seatbelt pretensioners to deploy inadvertently while the vehicle is being operated.
Inadvertent deployment of the air bags or the seat belt pretensioners increases the risk of an injury or crash.
Toyota will notify their owners. Pontiac Vibe owners will be notified by GM. Dealers will replace the air bag control module, free of charge. The recall began on August 24, 2015. Toyota is recalling certain model year 2003-2004 Corolla and Corolla Matrix vehicles and 2003-2004 Pontiac Vibe vehicles. The supplemental restraint system (SRS) circuits are susceptible to internal shorting. The electrical short may create an abnormal current flow and increased heat which can damage the circuits. This could result in an inadvertent deployment of the front airbags and/or seat belt pretensioners.
Inadvertent deployment of the airbags or the seat belt pretensioners may increase the risk of injury and the possibility of a vehicle crash.
Toyota will re-notify their owners who have already had their vehicles repaired with a noise filter, but did not receive a new airbag control module, and General Motors will re-notify theirs. Dealers previously installed a supplemental harness containing an electrical filter between the airbag control module and its wire harness, free of charge. Dealers will replace the airbag control module, free of charge. The recall began on March 13, 2013. The manufacturer has not yet provided a schedule for second notices. SOMETIMES, DRIVER-SIDE SEAT BELT GETS STUCK WHILE RETRACTING IT FROM THE ANCHOR POINT. I HAD TO TRY TWICE OR THRICE BEFORE THE SEAT BELT RETRACT SMOOTHLY FROM THE ANCHOR POINT. IT ALSO HAPPENS WHENEVER I TRY TO ADJUST THE SEAT BELT AFTER IT IS FASTENED TO THE CAR SEAT BUCKLE. DATE MENTIONED IN THIS FORM IS THE LATEST DATE WHEN I EXPERIENCED THIS PROBLEM.

THE CAR ALSO HAS STRUCTURAL PROBLEMS. WATER POURS THROUGH THE ROOF ONTO THE DRIVER WHEN IT RAINS. IT IS NOT DRIPPING; IT IS POURING WATER AND IT IS AN OBSTRUCTION TO THE DRIVER. I HAVE GONE BACK AND WORTH WITH THESE PEOPLE, BUT THEY DO NOT CARE ABOUT FEDERAL OR STATE LAW. THIS CAR NEEDS TO BE REPOSSESSED AND BE TAKEN OFF THE ROADS AND THIS RENT A WRECK LOCATIONS NEEDS TO BE MADE AWARE THAT THEIR BUSINESS TACTICS CANNOT PROCEED. MY TOYOTA COROLLA LE (CAR) COMPLETELY LOSES POWER AND SHUTS DOWN RANDOMLY. IT HAS HAPPENED ON NUMEROUS OCCASIONS. DURING ONE OCCASION IT SHUT DOWN WHILE ON THE HIGHWAY. THIS INCIDENT ALMOST COST ME MY LIFE AS WELL AS TWO OF MY CHILDREN (WHO ARE ADULTS). IT COMPLETELY SHUT DOWN IN THE MIDDLE OF A LOT OF TRAFFIC CAUSING A SEMI TRUCK TO MISS US BY INCHES. THIS HAS ALSO HAPPENED ON NUMEROUS OCCASIONS ON STREETS AND IN PARKING LOTS. THE CAR IS ALWAYS IN MOTION WHEN THIS OCCURS. I HAVE HAD THE CAR'S ENGINE TESTED (THE BUSINESS THAT TESTED IT IS NOW OUT OF BUSINESS). THE TEST REVEALED THAT IT WAS A COIL PACK THAT WAS CAUSING THE PROBLEM. I HAD THE COIL PACK REPLACED BUT IT DID NOT FIX THE PROBLEM. AT THIS POINT IT IS STILL UNKNOWN WHY MY VEHICLE SHUTS DOWN RANDOMLY. BELOW IT ASKS FOR A DATE WHEN THIS OCCURS. AS STATED ABOVE, IT HAS HAPPENED SO MANY TIMES THAT I CAN'T RECALL THOSE DATES, SO I PUT IN AN APPROXIMATE DATE THAT THE INCIDENT ON THE HIGHWAY OCCURRED. "TAKATA RECALL" THIS COMPLAINT IS NOT ABOUT THE AIRBAG ITSELF. IT HASN'T BEEN DEPLOYED. THIS IS ABOUT LUSTINE TOYOTA DEALER IN WOODBRIDGE, VA. I MADE AN APPOINTMENT FOR THE AIRBAG RECALL. SHOULD HAVE TAKEN ONE HOUR. AFTER TWO HOURS OF WAITING AND PEOPLE WORKING THERE TRYING TO SELL ME ANOTHER CAR, THEY SAID THEY HAD NO PARTS TO FIX IT AND TOLD ME TO COME BACK AT ANOTHER TIME. A FEW WEEKS LATER I MADE AN APPOINTMENT AND WENT IN AGAIN AND WAITED TWO HOURS AND THE SAME THING HAPPENED AGAIN. I GOT A CARD FROM THE MAN THERE WHO WAS WORKING WITH ME AND IT SAYS HE'S A SALESMAN NOT A MECHANIC. I NOTICED THIS AFTER I COMPLETED THE SECOND APPOINTMENT. BEFORE THE APPOINTMENTS I TOLD THE PERSON THE REASON I WAS COMING IN. I WILL TRY ANOTHER DEALER AFTER I FINISH FILING THIS COMPLAINT.

I BROUGHT MY CAR TO TOYOTA DUE TO A RECALL (SRA, ECU). AFTER TOYOTA PERFORMED WORK THE SEAT BELT AUDIBLE ALARM DOES NOT FUNCTION AND IS THEREFORE DANGEROUS. TOYOTA COULD NOT FIND PROBLEM AFTER SPENDING SIX HOURS. DRIVING WITHOUT FUNCTIONING SEAT BELT ALARM IS DANGEROUS. I HAVE COPIES OF TOYOTA DEALER WORK ORDERS. TAKATA RECALL I TOOK MY CAR IN FOR THE RECALL AND WAS TOLD A FEW WEEKS LATER THAT IT WAS COMPLETE. WHEN I WENT TO PICK UP MY CAR, HE TOLD ME THAT IT WAS ONLY PARTIALLY DONE, BUT WAS "COMPLETELY SAFE". HE WENT ON TO EXPLAIN THAT THEY REPLACED THE ONE PART THAT CAUSED THE AIRBAGS TO DEPLOY, SO NOT TO WORRY ABOUT THE SAFETY ANYMORE. I JUST RECEIVED THE RECALL AGAIN SAYING THAT THE PARTS WERE AVAILABLE, AND THAT IT WAS IMPORTANT TO GET IT FIXED BECAUSE THE AIRBAGS ARE STILL NOT SAFE. I AM LIVID...I DRIVE THIS CAR DAILY, AND HAVE MY CHILDREN IN IT AS WELL. A YEAR AND A HALF LATER?!! COMPLETELY UNACCEPTABLE.

I LOOKED UP MY VEHICLES RECALLS AND NOTICED THAT THERE IS ONE RELATED TO AIRBAGS. COINCIDENTLY I DO HAVE AN AIR BAG LIGHT LIT ON MY CAR AND IT SEEMS LIKE MY MODEL YEAR IS SUSCEPTIBLE TO SHORTS IN THE SYSTEM. I CONTACTED THE DEALER AND THEY TOLD ME THAT THEY HAVE A 6 WEEK OR MORE WAIT ON THE RECALL AND THEY WILL CONTACT ME. I FIND THIS TO UNACCEPTABLE FOR A SAFETY FEATURE.

ON 8/16/16 A 2004 TOYOTA COROLLA REAR ENDED A DODGE RAM VAN AT APPROX. 65 MPH ON EASTBOUND INTERSTATE 275 AT THE 48.5 MILE POST. THE INVESTIGATION SHOWS THAT ALL OCCUPANTS WERE WEARING THEIR SEAT BELTS, HOWEVER THE RIGHT REAR OCCUPANT SUFFERED LIFE-THREATENING INJURIES TO HER LIVER AND SPLEEN. THE SEAT BELT SHOWS SIGNS OF BEING WORN BUT THE RETRACTOR DID NOT LOCK. I THINK THAT THE SHOULDER STRAP PORTION MAY NOT HAVE BEEN WORN PROPERLY DUE TO THE WAY THE SEAT BELT WAS FOUND CINCHED UP INTO THE CLASP (PICTURES ATTACHED.) CAR WAS RUNNING PERFECTLY. VERY SUDDENLY IT STARTED MAKING THIS HORRIBLE GRINDING NOISE. I WENT TO PULL INTO A PARKING LOT AND THE CAR JUST STOPPED. I WAS TOLD THAT THEY DID NOT HAVE THE PART AND THERE IS A WAITING LIST FOR THE ACTUAL PART. I HAVE CALLED TOYOTA SEVERAL TIMES JUST TO HEAR THAT THEY ARE STILL WAITING FOR THE PART AND THEY WILL CALL ME ONCE IT IS IN. IN THE MEANTIME, MY CAR IS PART OF THIS RECALL AND NO ONE HAS TRY TO FIX THE PROBLEM.

I CALLED THE DEALER JULY 11 AND WAS TOLD THE SRS PART WAS NOT AVAILABLE AND IT COULD BE SOMETIME MAYBE MONTHS BEFORE THE PARTS WOULD BE AVAILABLE. HE STATED IT WAS TAKATA'S FAULT AND THERE WAS NOTHING THE DEALER COULD DO. I WOULD LIKE TO KNOW IF WHAT THE DEALER IS TELLING ME IS CORRECT.

I TRIED TO CALL THE NEAREST TOYOTA DEALERSHIP AND THEY COULDN'T EVEN SCHEDULE THIS APPOINTMENT OVER THE PHONE. THEY SAID THEY WERE BUSY AND WOULD HAVE TO HAVE AN "ADVISER" CALL ME BACK. I WILL NEED A RENTAL CAR PAID FOR IN ORDER TO GET THIS SERVICE DONE. I WOULD THINK THAT THIS WOULD THE UTMOST OF IMPORTANCE AND SYSTEMS/PERSONNEL WOULD BE IN PLACE TO HANDLE THIS MORE EFFICIENTLY AND MORE TIMELY. SINCE THIS IS THE MANUFACTURERS' NEGLIGENCE, I EXPECT TOYOTA TO DELIVER ON IT'S LEGAL COMMITMENT TO PAY FOR ALL COSTS ASSOCIATED WITH THIS BURDEN.
