    I am reporting a severe safety defect regarding a failed engine coolant bypass valve on my 2022 Toyota Corolla Apex SE (VIN: [XXX], Mileage: 54,700). An authorized Toyota dealership has scanned the vehicle and confirmed active Diagnostic Trouble Code (DTC) P268115 ("Engine Coolant Bypass Valve Circuit Open"). This mechanical and electrical failure triggers an immediate dashboard warning and creates an imminent risk of sudden engine overheating, thermal engine damage, and catastrophic loss of motive power (stalling) while traveling at highway speeds, placing myself and other motorists at risk.Toyota is fully aware of this widespread defect and issued Customer Support Program (CSP) 24TE04 to extend the warranty on older models (2019–2021) for 10 years or 100,000 miles due to high part failure rates. However, my February 2022 vehicle was factory-equipped with the exact same defective, un-superseded valve part number, yet Toyota has arbitrarily excluded my vehicle from free repair based solely on its calendar production date.Furthermore, Toyota's corporate office has denied goodwill coverage because I am a secondhand owner without previous maintenance records, despite this being a sealed electrical actuator failure completely unrelated to routine maintenance. I am a [XXX] senior citizen living on a fixed Supplemental Security Income (SSI) budget. Toyota's denial forces an elderly, low-income consumer to either navigate a hazardous, stalling-prone vehicle on public roads or lose essential transportation because an arbitrary corporate policy places a $1,045 manufacturing defect repair entirely out of financial reach.     The contact owns a 2022 Toyota Corolla. The contact stated that while driving at 50 MPH, the vehicle was hard to stop, and he lost control but was able to avoid a crash. The maintenance light was illuminated. Upon further research, the contact indicated the failure was related to TSB T-SB-0088-23. The vehicle was taken to the local dealer, where it was diagnosed and determined that the engine mount had cracked. The engine was hanging from the vehicle. The vehicle was not repaired. The vehicle was taken to an independent mechanic who had repaired the vehicle. The manufacturer was notified. The failure mileage was approximately 80,000.

    Vehicle won't move. Emergency break malfunction notification came on. Unable to get to dealership without a tow. Diagnostic completed.. Quick service provided $701.66. Two A21 Cylinder Assy, RR DI @$402.90. Two A11 Actuator assy, parking @ $2168.34 also customer paid shop charge for repair order @ $99.00. Plus tax Total $3585.52 This car is a 2022 toyota corolla hybrid. Mileage at 78,000. This on flat ground in clients driveway, and the in emergency break was not used after turned off. Was inform is that the brakes looked in very good condition, so wonder why the emergency brake malfunction notification came on with Vehicle unable to move. Why was not explained.

    The component that failed is the coolant bypass valve, which is part of the engine cooling system. The vehicle is available for inspection upon request. The issue was first indicated by an "Engine Maintenance Required" warning message that appeared on the dashboard. No prior symptoms were noticeable before the warning appeared. The warning came on suddenly during normal driving conditions. This failure poses a potential safety risk because the coolant bypass valve regulates engine temperature. If it malfunctions, it may lead to improper engine cooling, overheating, reduced engine performance, or sudden breakdown while driving. This could increase the risk of being stranded in traffic or experiencing a hazardous situation on the road. The problem has been diagnosed and confirmed by an authorized Toyota dealership, which identified the coolant bypass valve as failed and in need of replacement. The dealership provided a repair estimate of $775 prior to taxes and fees. The manager shared that it would be around $890+ after it is all calculated. The vehicle has not been involved in any accident related to this issue, but the malfunction was verified through dealer inspection. No police or insurance reports were filed, as this was a mechanical failure. Based on personal research, this appears to be a widespread issue affecting similar Toyota vehicles, including other Corollas and RAV4 models, where the same component has failed under similar conditions. In some cases, repairs have been covered by the manufacturer, indicating awareness of the issue. There are many groups on Facebook and Reddit where customers with the same make and model 2022 Toyota Corolla SE have had the same issue after 3 years of having the vehicle. In summary, while driving under normal conditions, the warning message appeared without prior indication. The dealership later confirmed that the coolant bypass valve had failed and required replacement.

    Dear National Highway Traffic Safety Administration, I am submitting this email to formally report a potential vehicle safety defect involving my 2022 Toyota Corolla. Vehicle information: - Make/Model: 2022 Toyota Corolla - In-service date: 08/18/2022 My vehicle sustained significant wiring damage caused by rodents. Toyota has denied warranty and financial assistance, characterizing the issue as an “outside influence.” However, according to what I read online, modern vehicles —including Toyota models—use soy-based or bio-based wiring insulation, which is widely documented to attract rodents. The damage affected critical electrical wiring, creating potential safety hazards including loss of vehicle function and increased fire risk. This is not merely cosmetic damage but impacts essential vehicle systems. The foreseeability of rodent attraction due to wiring materials raises concerns of a design-related safety defect and failure to adequately warn consumers. Toyota has stated there are no recalls or customer support programs addressing this issue, despite recurring reports from vehicle owners nationwide. I respectfully request that NHTSA review this matter as a potential safety defect and consider whether further investigation is warranted. Thank you for your attention to this safety concern.     The radiator had a severe pin hole in several veins causing coolant to leak out and making the car overheat. The overheating seems to happen over a long distance drive jeopardizing my safety and family. To prevent futher damage to the car and my safety. Yes. I have to wait for hours on the side of the highway for hours-to let the car cool down and add recommended coolant for the car. The vehicle was inspected by TOYOTA of San Louis Obispo. 09/29/25. I am not certain if the vehicle was inspected by the manufacturer but the service repair was done @ Toyota dealersip. The temperature or the coolant gauge moved erratically.

    The contact owns a 2022 Toyota Corolla. The contact stated while driving 55 MPH, the vehicle was losing motive power. The contact depressed the accelerator pedal, and the vehicle failed to respond. The contact stated that prior to the failure, the TPMS warning light was illuminated. The contact stated that the engine failure was intermittent but had recurred at various speeds. Additionally, the contact stated that the battery was drained, and needed to be jumpstart the vehicle more than twice. The contact had taken the vehicle to a local dealer however, the vehicle had not been diagnosed. The vehicle was not repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 27,000.

    Car stalled at stoplight. Loud tone sound with ‘power steering low’ indicator with check engine light on. Wheel locked. Needed to put in park and restart car. Very scary and unsafe! Multiple people on Reddit and YouTube have reported this!

    There has been several instances where I’m trying to cross a divided Highway and the accelerator doesn’t respond. I’ve had three near death experiences. There’s been other times where it will just shut off and restart without a warning. It’s been in my driveway thankfully. When I pull out in front of someone, it doesn’t respond either. I have had the pedal all the way down to the floor and it wouldn’t go for about 5-6 seconds
