Representative owner complaints: forward collision avoidance

180 reports, newest first. Show all

  1. Forward collision avoidance, Lane departure, Mar 27, 2026

The front camera that controls adaptive system died within 4years of owning the vehicle so all the warning lights come on now.

  2. Forward collision avoidance, Lane departure, Mar 26, 2026

The Multipurpose Camera failed. Searches online and discussion with the dealers show this is a common problem. Result is shutdown of collision mitigation systems, blind spot indicators, lane divergence system, auto high beam, etc.

  3. Lane departure, Forward collision avoidance, Mar 16, 2026

The multipurpose camera unit failed causing the Forward Collision Warning, Automatic Braking, Lane Keeping Assistance, and Lane Departure warnings lights to come on and rendering all those systems inoperable. Part of the selling point of this vehicle is these additional safety features and for them to fail within 5 years of owning the vehicle is a safety concern. From my own research online, it seems this is a common problem for Odysseys built around this timeframe. This part is extremely expensive and is a required repair if you want the original safety features for the vehicle. I made the repairs, but the life of these units is something that should be looked into.

  4. Forward collision avoidance, Lane departure, Mar 15, 2026

System failure for all of the safety-related assistants. Works intermittently, but mostly indicates failure.

  5. Forward collision avoidance, Lane departure, Mar 13, 2026

Getting error code for safety sinsing. Lane keep assist, Collision mitigation system and now transmission system. forward facing camera is cause of issue

  6. Electrical system, Forward collision avoidance, Mar 10, 2026

The multipurpose camera module has failed, on the vehicle resulting in the simultaneous loss of several driver-assistance and safety systems while the vehicle is in operation. These systems include cruise control, lane keep assist, lane departure warning, collision mitigation, automatic high beams, and the brake system warning. Upon inspection, there appears to be overheating within the camera module. This overheating has caused visible discoloration both inside and outside the module housing. Based on these observations, I believe the condition may present a potential fire hazard. Additionally, the plastic cover mounted to the windshield becomes extremely hot to the touch, creating a risk of skin burns if it is accidentally contacted. This malfunction poses a significant safety concern for my family. If the camera fails while driving, the vehicle’s safety systems may not function as intended, increasing the risk of an accident. All of the failures chime in one after another displaying a problem with the malfunctioning components stated earlier in the text. Furthermore, if a driver is deaf or not actively monitoring the instrument panel or speedometer, they may not hear or immediately notice the warning alerts indicating that these systems are no longer functioning. The issue has been diagnosed by my local repair shop finding error code U3000-49: Camera/Radar Internal Failure. Repair requires replacement of the multipurpose camera module, with an estimated cost exceeding $2,000 in parts and labor, which represents a significant financial burden.

  7. Forward collision avoidance, Feb 26, 2026

The front-facing camera stopped working, and all associated ADAS systems no longer functioned (Adaptive Cruise Control, Automatic Emergency Braking, Forward Collision Warning, Lane Departure Warning, Lane Keeping Assistance). Several errors needed to be cleared every time the vehicle was started. Our Honda dealership determined that it was a failed Multipurpose Camera (code U3000-49), and that the entire camera unit needed to be replaced and re-calibrated.

  8. Forward collision avoidance, Lane departure, Feb 25, 2026

System is starting to fail by alerting me to clean windshield. Windshield and camera have been cleaned, still getting multiple warnings that system is shutting off safety features during my commutes.

  9. Electrical system, Forward collision avoidance, Feb 3, 2026

Multiple Honda Sensing safety systems (collision mitigation, lane keeping, road departure, adaptive cruise, and auto high‑beam) disabled suddenly on 2020 Honda Odyssey EX‑L around 76,000 mi. Dealer and two independent diagnostics confirmed DTC U3000‑49, internal electronic failure of the forward recognition camera. The fault disables all automatic braking and lane‑keeping functions. Battery voltage and calibration verified OK; component failure is internal. Honda quoted ~$2,000 repair. This is a significant safety risk since all advertised crash‑avoidance functions stop working.

  10. Forward collision avoidance, Feb 3, 2026

the front monocular camera is defective causing the safety features (adaptive cruise control, collision warning, lane shift warning, auto brights, auto braking) to not function at all. This seems to be a known issue with many honda owners. The warnings show every time I start the van. This issue has been verified and the repair is costly.

  11. Lane departure, Forward collision avoidance, Jan 26, 2026

After purchasing my 2020 Honda Odyssey EX-L in July 2025 (had approximately 58,000 miles), in January 2026 (current mileage approximately 66,000 miles), it suddenly displayed multiple indicators on the dash, including 'ACC', 'LKAS', 'Brake System', 'Auto High Beam Problem', and 'Collision Mitigation'. My local mechanic indicated the cause was the collision radar system, but it needed to be calibrated at a Honda dealership. My local Honda dealership indicated the problem code as 'U3000-49 Internal Camera Malfunction', necessitating that the multi-purpose camera be replaced. The total repair cost was $2,104.99. Online research shows that this is a common issue with this model year. If the cause is due to a camera malfunction or disrepair, and it impacts the safety of myself and others in the vehicle, why is this not considered a valid reason for a recall? I shouldn't be responsible for a costly repair that didn't result from any mechanical issue or mistreatment on my part. Because of the safety implications, this should be a Honda recall, and I should be reimbursed for the repair cost.

  12. Forward collision avoidance, Lane departure, Jan 22, 2026

Went to pull into parking lot, ADAS system starting going crazy. Throws transmission malfunction error. Headlights won’t work - they turn on and off or go extremely dim. Van keeps jerking and losing power. Code reader showed U3000-49 electronic failure. Van drove normal, no issues until this code popped up. Issue started 01/08/2026.

  13. Forward collision avoidance, Lane departure, Jan 16, 2026

The Multipurpose Camera Unit has failed resulting in loss of Adaptive cruise control, lane keep assist, Automatic braking and other features related to the MCU. There are approximately 6 warnings when starting the car of features not working, and 3 lights that remain lit on the dash including "Brake System". This is the second time this issue has arise. The first time was replaced by the dealer under warranty.

  14. Forward collision avoidance, Jan 15, 2026

I am submitting this complaint to report a serious safety defect involving my Honda Odyssey equipped with Honda Sensing technology. During normal operation and without any accident, impact, windshield damage, repair, or environmental event, all Honda Sensing safety systems suddenly and simultaneously stopped working. The disabled systems include Collision Mitigation Braking System, Adaptive Cruise Control, Lane Keeping Assist, Road Departure Mitigation, Forward Collision Warning, and related camera- and radar-based features. Multiple warning messages appeared, and the vehicle effectively lost all advanced driver-assistance and collision-avoidance capabilities at once. This occurred without warning and without any action on my part. This failure is extremely concerning because Honda Sensing features are marketed and relied upon as critical active safety systems intended to help prevent or mitigate crashes. A sudden, total loss of these systems materially reduces vehicle safety and places occupants and other road users at increased risk. A safety system that can fail entirely and unexpectedly—absent any triggering event—indicates a potential systemic defect, not routine maintenance or wear. I am concerned this issue may affect other Honda vehicles equipped with Honda Sensing and may be related to a common failure in cameras, radar units, software, or vehicle electronics. I respectfully request that NHTSA review and log this complaint, investigate this failure pattern, and determine whether a defect investigation, service bulletin, or recall is warranted in the interest of public safety. Vehicle details: Make: Honda Model: Odyssey Model Year: 2020 Mileage: 87000 VIN: [XXX] Thank you for your attention to this serious safety matter. 15. Forward collision avoidance, Lane departure, Jan 15, 2026

My Multi-Purpose Camera System has gone bad after only 5 years. Collision mitigation, cruise control, brake system, lane keep assist, among other safety features are failing because of this.

  16. Service brakes, Forward collision avoidance, Jan 13, 2026

Multiple warning lights, all for the safety system: ACC, automatic emergency braking, etc. I am told by the dealer that this is due to a faulty forward camera. Looking at the NHTSA site and other forums, this is a VERY common issue and should be addressed.

  17. Forward collision avoidance, Lane departure, Jan 13, 2026

The Multipurpose Camera Unit has failed resulting in loss of Adaptive cruise control, lane keep assist, Automatic braking and other features related to the MCU. The dealer inspected the car and confirmed the issue was a bad camera. There are approximately 6 warnings when starting the car of features not working, and 3 lights that remain lit on the dash including "Brake System".

  18. Forward collision avoidance, Lane departure, Jan 12, 2026

Upon start up my 2020 Honda Odyssey EX-L throws a slew of pop-up’s on the dash (collision mitigation problem, auto high beam problem, adaptive cruise control problem, lane keep assist problem, road departure mitigation problem). It's been doing this for months, but when it started, I had just gotten an oil change 2 weeks prior and knew my mechanic would've caught this if it was something major. Last week I took it back in for another oil change and asked them to check these errors. They checked the battery and it’s fine (under 2 years old), but they confirmed it's the adaptive cruise control camera. They said not many places work on those cameras and I'll have to have the dealer do the repair. Well upon searching, it seems pretty consistent that this repair is costing owners $1700 because the camera has to be recalibrated with the software after it's replaced. I'm also seeing this problem appears to be widespread for Honda owners. Honda needs to do something to make this right for their customers. A $1700 repair for a camera that randomly dies by no fault of the owner is insane, and considering how many driver assistance systems to improve safety are affected, Honda should be held accountable. 19. Electrical system, Forward collision avoidance, Dec 29, 2025

FEDERAL SAFETY RECALL 23V-431 FAILED AFTER FOUR REPAIR ATTEMPTS - MULTIPLE SAFETY SYSTEMS NOW NON-FUNCTIONAL My Honda was subject to Federal Safety Recall 23V-431 (MOST Communication Cable causing backup camera failure). After FOUR repair attempts, the original defect persists AND multiple additional safety-critical systems are now failing. REPAIR HISTORY: August 7, 2023 (30,749 miles) - Honda San Carlos performed recall repair 23V-431. Dealer claimed issue resolved. October 5, 2024 - Original backup camera/infotainment failure returned. I documented with photo evidence but problem was intermittent. September 29, 2025 (48,038 miles) - Returned to Honda San Carlos. Same backup camera failure. Dealer attempted repair. November 18, 2025 (48,928 miles) - Returned to Honda San Carlos. Problem persists. Dealer claimed repair complete. December 20, 2025 - Returned to Honda San Carlos. Original problem PLUS multiple NEW safety system failures now appearing: Backup camera (original recall issue) - STILL FAILING Adaptive Cruise Control - ERROR Collision Mitigation System - ERROR Lane Keep Assist System - ERROR Auto High Beam System - ERROR Front camera - ERROR DEALER RESPONSE: Honda San Carlos now claims new failures are "unrelated" to recall 23V-431 and proposes $2,900 for front camera replacement. I declined as all failing systems communicate via the MOST network that was subject to the original recall. SAFETY CONCERN: I now have a vehicle with MULTIPLE non-functional federally-mandated and safety-critical systems due to what appears to be the same MOST communication defect that recall 23V-431 was supposed to fix. The recall remedy has demonstrably failed and the problem is worsening. Honda's recall remedy is inadequate and has resulted in cascading safety system failures. Vehicle is unsafe to operate with non-functional collision mitigation, lane keep assist, and camera systems.

  20. Service brakes, Forward collision avoidance, Dec 23, 2025

Multi-Purpose camera has failed according to Honda dealership. No safety features work in the car. You pay for the car to have these features and then they fail after only 3 years? Have been dealing with this issue for almost 2 years now. Honda wants $1,800 for the new camera. This is a safety issue that Honda should be responsible for fixing. No adaptive cruise control, no lane keeping assist, no auto headlights, no collision mitigation, no blind spot alarms.

  21. Forward collision avoidance, Lane departure, Dec 15, 2025

In December, out of the blue suddenly had rotating list of warning on dash panel (Lane Keeping Assist problem, Collision mitigation system, departure mitigation system problem, auto high-beam problem, Adaptive cruise control problem) plus Brake Warning system light is on constantly and brake hold not working. At first this was intermittent but now on constantly when starting and driving car. Only 55k miles on my Odyssey. Dealer said he had 3 of these in the last three months. Said it was Multi Purpose front camera failure. Cost over $1500. Since huge safety issue putting not only my family but other cars/people at risk, we are paying to repair, however we just found hundreds of people complaining about this same issue. This should be a Honda Recall. We did nothing wrong to cause this safety issue and this part should NOT fail in 5 years. I don't have the final invoice yet but I can upload later if required

  22. Forward collision avoidance, Lane departure, Dec 10, 2025

The front camera that regulates the adaptive cruise control failed. Now it does not allow for any of the safety features to work. I’ve gone on multiple Honda forums and a lot of people have the same issue.

  23. Lane departure, Forward collision avoidance, Dec 3, 2025

After researching I find it to be a sensor in the windshield is a common problem

  24. Forward collision avoidance, Lane departure, Nov 28, 2025

The forward facing camera failed on my 2020 Odyssey with only 90,000 miles. Because of this failure, I am loosing all my forward facing safety features. This seems to be a common issue across many Honda vehicles and most users report being told the issue is being caused by overheating.
