  1. Power train, Vehicle speed control, Forward collision avoidance, Jul 31, 2026

My 2022 Honda Pilot Black Edition (VIN [XXX], 62,900 miles) has multiple intermittent safety‑critical failures involving the transmission, acceleration, adaptive cruise control, and HVAC blower. These issues occur without warning and without diagnostic trouble codes. Transmission/Acceleration Failure: Most recent incident was XXX on Highway 71 during heavy traffic. The vehicle twice experienced rapid deceleration with no ability to reach a safety stop. RPM dropped below 1000, then spiked to 6,000–8,000 while the vehicle continued to lose speed. Acceleration returned only after several seconds. This also occurs during highway merging and uphill acceleration. Additionally, when disengaging cruise control and immediately pressing the accelerator, the vehicle loses speed, RPM drops abnormally low, then spikes before power returns. Adaptive Cruise Control/Honda Sensing Failure: On XXX the vehicle rapidly decelerated when a semi‑truck was beside me, despite no vehicle ahead. The week prior, while driving several states away, setting cruise at 70 mph caused sudden deceleration and cruise resetting itself to ~54 mph. These events resemble phantom braking and radar mis‑targeting. HVAC Blower Failure: During a long multi‑state trip the week prior, the AC blower (speed 3–4) intermittently stopped blowing for several seconds, then resumed at the same speed with no change to the display. These failures affect powertrain, forward collision avoidance, vehicle speed control, electrical systems, and cabin equipment. The dealership (Roush Honda, Westerville OH) cannot replicate the issues and reports no stored codes. These defects pose significant safety risks. 2. Electrical system, Jul 13, 2026

Vehicle failed to restart after auto start stop system was activated at a red light.

  3. Steering, Jul 13, 2026

Bought brand new, I am at almost 68,000 miles. I have had vibration, rattling, etc near the front left of the car. After inspections and diagnostics. They said I need a new steering rack. It seems pre-mature to need a steering rack. No damage or high impact things have happened. Please investigate this issue to see if it could be manufacturing issue. Thank you! I am having it repaired by next week, if you need a receipt please email me at [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

  4. Service brakes, Hydraulic, Vehicle speed control, Jul 10, 2026, crash

The contact owned a 2022 Honda Pilot. The contact stated that while the vehicle was parked in a parking spot with the brake pedal depressed, the engine revved unexpectedly and independently accelerated and crash into a Ford Bronco, a Jeep Cherokee, another vehicle, and two parking signs. There were no warning lights illuminated during the failure. The contact had made three other stops prior to the crash. There was no injury sustained. The air bags deployed as needed. A Police report was filed. The vehicle was towed to a tow lot. The local dealer was contacted, and the contact was informed that the failure was possibly caused by a stuck floor mat. The dealer declined to inspect the vehicle. The vehicle was not diagnosed or repaired. The manufacturer was not contacted. The insurance company was contacted and deemed the vehicle a total loss. The owner had not yet released the vehicle to the insurance company. The failure mileage was approximately 56,000.

  5. Electrical system, Service brakes, Forward collision avoidance, Jul 8, 2026

Brake light wont cut off and drains battery. Car wouldn't start and once battery was charged all steering, cruise control, hill assist, lane assist and other lighs came on. 7 total alerts came on and car struggled to go up hills and speed up after braking at lights. Brake light stayed on entire drive and didnt go off even when the car was off.

  6. Power train, Jul 6, 2026

A safety concern with some Honda Pilot models involves a potential transmission defect that can cause the vehicle to hesitate or fail to accelerate when the driver presses the accelerator. This unexpected loss of power can create hazardous situations, particularly when merging into traffic, crossing intersections, or attempting to avoid obstacles. Drivers have reported delayed engagement and inconsistent acceleration, raising concerns about the reliability and safety of the vehicle's transmission system.

  8. Other or unspecified, Jul 2, 2026

This week I was leaving an event to go home and was driving on a fairly busy local Road came to the stoplight. The light was red, came to a complete stop and waited for the light to turn green. When the light turned green I pressed my gas pedal and my vehicle stalled on me. It completely turned off. I had to reset my vehicle two times in order to get it started again so that I could make it to my next destination and by resetting it I had to power the vehicle off. I have a push to start vehicle so I have to power the vehicle off and then open my door, close my door then try to start my vehicle again with a push to start. That did not work so I had to do it again the same process and then my vehicle started. I had to then turn on the the button press the button to turn auto idle off. That is how my vehicle was able to drive and not stall on me again. I forgot to turn off that button on my initial trip and that's when my vehicle stalled.

  9. Other or unspecified, Fuel/propulsion system, Lane departure, Jun 25, 2026

I was driving home at 4am on I-95 with my children in the car. My car has had a history of "juttering" and making a vibrational noise, but on the way home there was an issue with acceleration when using the gas pedal, and slow to gain speed. There were times when the gas pedal didn't respond to my foot depression at all, and did not accelerate. When I was almost home, many lights came on my dash including: Blind Spot warning, emissions systems problem, transmission problem apply parking brake when parked, stop driving when safe transmission problem see manual. The car is not driveable at this time and sitting in my driveway. I have no idea where to tow it to or what to do or how much it will cost

  10. Forward collision avoidance, Lane departure, Jun 18, 2026

Windshield cracked majorly after being hit by a small rock on the highway. Even after having it replaced and the camera recalibrated, 7 safety warnings of the ADAS come on. Reportedly, the frontcamera has failed, which is concerning in a car that has less than 50k and is just about 4yrs old. I believe this is a larger issue, the windshield truly should not have broken that easily and the camera failing so soon is problematic. It is also a very expensive fix.

  11. Vehicle speed control, Jun 15, 2026, crash

The contact owned a 2022 Honda Pilot. The contact stated that while reversing into the garage, the accelerator pedal was depressed and the vehicle continued to accelerate before crashing into the garage. On another occasion, while driving into the parking lot, the brake pedal was depressed; however, the vehicle independently accelerated and crashed into a store. There were no warning lights illuminated. The driver was not injured but sought medical attention at the hospital and was treated for three days. A police report was filed. The air bags did not deploy. The vehicle was towed to a tow lot. The contact was informed by a friend that the failure was related to the throttle body. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The vehicle was towed to an unknown location and deemed a total loss by the Insurance provider. The failure mileage was unknown. The VIN was not available.

  12. Other or unspecified, Jun 2, 2026

Driving on freeway, at speed limit. All was clear. The sunroof spontaneously exploded. Nothing struck it, it just exploded on its own.

  13. Other or unspecified, Service brakes, Forward collision avoidance, May 11, 2026

“The vehicle experiences phantom braking while driving, where the Collision Mitigation Braking System (CMBS) activates without warning even when there are no vehicles or objects in front of me. The vehicle suddenly brakes on its own, creating a major safety hazard and risk of rear-end collisions. The issue has happened multiple times during normal driving at different speeds. Warning lights related to the collision mitigation system and passenger airbag system have appeared intermittently. The blind spot monitoring system has also failed to properly detect nearby vehicles at times. The vehicle was previously taken to the dealership/service department and remained there for an extended period for inspection and attempted repairs, but the problem was not fully corrected. After getting the vehicle back, the same symptoms and braking issues continued. This issue puts myself, my passengers, and other drivers at risk because the vehicle may suddenly brake unexpectedly in traffic.

  14. Other or unspecified, May 11, 2026

Our sunroof on our Honda 2022 Pilot spontaneously shattered while we were sitting at a stop light. Nothing fell on it and nothing hit, it just suddenly shattered while we were sitting in the stop light.

  15. Electrical system, Back over prevention, Forward collision avoidance, May 11, 2026, crash

My 2022 Honda Pilot failed to engage Drive after reversing, despite the Drive button being pressed. This resulted in an unintended backward motion & a collision with another vehicle. 1.What component or system failed or malfunctioned…? The Electronic Transmission Shift-by-Wire system (push-button shifter) failed to respond to a gear change request. Additionally, related electrical systems including the dashboard infotainment system & the integrated USB charging ports have malfunctioned. The vehicle & its components are available for inspection upon request. 2.How was your safety or the safety of others put at risk? The vehicle experienced unintended motion. While attempting to move forward in a parking lot, the vehicle remained in Reverse despite selecting Drive. This failure led to a low-speed collision with a truck’s tow hitch. This defect creates a high risk of crashes, injuries to pedestrians, or property damage because the vehicle does not move in the direction the driver has selected. 3.Has the problem been reproduced or confirmed by a dealer or independent service center? The vehicle was previously serviced at a Honda dealership for a radio/infotainment recall. Since that service, the transmission has failed to engage Drive multiple times at random. 4.Has the vehicle or component been inspected by the manufacturer…? The vehicle was inspected during the initial recall fix. And brought back because after the fix because the charging port stopped working/continued screen glitches. Following the recent collision, it is awaiting further inspection by a Honda dealership to determine the extent of the mechanical failure. 5.Were there any warning lamps, messages prior…? Symptoms began after the recall fix & included intermittent electronic failures of the charging ports/screen. There were no specific transmission warning lights prior to the gear selection failure; however, the persistent electrical issues in the dash area suggested a systemic electronic malfunction.

  16. Forward collision avoidance, Lane departure, May 10, 2026

I turned my car on one day and warning lights for all of my safety sensing features came up. Intermittently they work for a little bit. I brought it to the dealer, and they said that there was damage to the sensor, but there has been no damage to the front end of the car. Their reply was perhaps a pebble came up from the road while I was driving. It seems to me that that is a fault in the manufacturing if it is that easy for all of the sensing features to stop working. I attempted to sort this out with the dealer and with Honda corporate offices with no luck.

  17. Electrical system, May 5, 2026

This is the recall information on the Honda site. The failure of the backup camera on our vehicle is what is described in the recall, yet the Honda site does not include our vehicle. (Honda) is recalling certain 2019-2022 model year Pilot and 2019-2023 model year Passport vehicles. The Media Oriented Systems Transport (MOST) communication coaxial cable was improperly designed and manufactured, resulting in deformed terminals and potentially causing poor, or lost, connections intermittently between the audio display unit and the vehicle cable connector. Safety Risk If the MOST communication is lost for more than 20 seconds, the rearview camera function will fail when the vehicle is in the reverse gear, increasing the risk of a crash or injury. Remedy Registered owners of all affected vehicles will be contacted by mail and asked to take their vehicle to an authorized Honda dealer. The dealer will install an improved MOST cable harness between the existing display audio and vehicle terminal connections and a straightening cover over the vehicle cable connector to properly connect the audio display unit. Owners who have paid to have these repairs completed at their own expense may be eligible for reimbursement, in accord with the recall reimbursement plan on file with NHTSA. 18. Engine, Apr 24, 2026

On April 3,2026 we are starting back from a road trip to Florida from Kentucky (starting odometer 69,199 miles)oil changed prior to leaving in our 2022 Honda pilot. Stopped to get gas and transmission alert code popped up-it would not go into any type of gear. We had no warning previously to this. We were still 3 hours from home, had to get it towed to nearest Honda dealership where they stated it was an electronic module failure but transmission would have to be replaced entirely. Car was out of standard power-train warranty of 60,000 miles d/t this road trip (odometer now over 70K) and we had no extended warranty on the vehicle. We requested assistance for transmission replacement from American Honda. We filled out the required form on 4/9/26 explaining we are a loyal Honda customer(own an additional Honda-CRV), had good service records, etc. It was assigned to a case manager on Fri 4/10-they called left voicemail that it was under review and asked us to call back. We immediately called back on Friday 4/10 and left message-no call back. The following week we called back everyday to American Honda and finally got our case manager over a week later on Monday 4/20-said it would be 5 more days. Heard back today 4/24/26 that Honda denied our assistance request saying it was out of warranty and there are no known manufactorer defects or recalls at this current time. Made us wait 3 weeks to decide we will have to eat cost of a new transmission in a car that is less than 4 years old and <100K miles on it. Very disappointing in Honda corporation especially since there are numerous known complaints, civil law suits, etc on various online forums and in the national news of same issues and they aren't taking any responsibility leaving owners with very high repair expenses. Our transmission will be over $8000 to replace out of pocket.

  19. Service brakes, Apr 20, 2026

Each time my battery has died the brake starter has also stopped working. I have had to get the starter replaced three times in 4 years. The starter for the Honda pilot 2022 is faulty

  20. Power train, Engine, Fuel/propulsion system, Apr 20, 2026

Upon accelerating from a stop, or at various speeds, the vehicle experiences a significant loss of motive power(pulling backwards) followed by an abrupt lunge forward. This hesitation occurs unpredictably and makes entering traffic or intersections extremely hazardous. This feels like a fuel delivery or transmission engagement failure. The last incident occurred on or around the afternoon of March 23. My vehicle has been examined at the dealership on multiple occasions for this same issue and each time I have been told that they are unable to replicate the issue and can not help me further unless they are able to replicate the issue.

  21. Engine, Fuel/propulsion system, Apr 4, 2026

While driving on the expressway at speeds above 60 mph and while attempting to pass another vehicle the vehicle shuts down with no acceleration. The vehicle will then after several seconds regain acceleration. Several times it has sounded like the vehicles starter engaged. This is a random occurrence and has happened approximately 10 times. There are no error codes and the dealership states that they can not recreate the issue so there is nothing wrong.

  22. Electrical system, Vehicle speed control, Engine, Apr 2, 2026, crash

I was operating a 2022 Honda Pilot and had just made a left turn into a parking lot. I was slowly pulling into a parking space directly in front of a dance studio with a large floor-to-ceiling window. There were four children and a dance instructor standing inside near the window. As I was coming to a stop to park, my foot was on the brake. At that moment, the engine suddenly revved loudly and the vehicle accelerated forward unexpectedly without any additional throttle input. There were no warning lights, messages, or prior symptoms before this occurred. Despite braking, the vehicle continued forward and struck the exterior of the building. I was able to avoid the window area where the children were standing. The airbag deployed, and the front passenger side of the vehicle was damaged. The building was not structurally damaged, and no one was injured. There were no obstructions such as floor mats interfering with the accelerator, and there was no unintended pedal input. My foot was on the brake at the time of the incident. This appears to be a case of sudden unintended acceleration. The issue has not been reproduced or confirmed by a dealer or service center, and there were no prior warning signs before the event. The vehicle is available for inspection upon request. It has been towed to the body shop. The problem has not been reproduced or confirmed by a dealer or independent service center at this time. The vehicle was inspected by insurance and they did not total it. It is going to be repaired. The vehicle has been inspected by insurance representatives. The manufacturer has not inspected the vehicle at this time. I did not call police because I was not hurt, the kids were not hurt and the building only had a couple of scratches on the brick.

  23. Electrical system, Engine, Mar 21, 2026

When the vehicle is stopped at a light and the auto engine idle engages, it intermittently stalls or shuts off the engine and unable or struggles to restart. The most recent time, the Pilot had to be shut off completely and restarted while sitting at a newly turned green light with large semi trucks behind us. When the auto engine idle is turned off there are no problems. This has happened 5 times over the course of the last 2-3 months.
