As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 108, "Lamps, Reflective Devices, and Associated Equipment."
Headlight low beams that are too bright can reduce visibility for oncoming drivers, increasing the risk of a crash.

The FSD Beta system may allow the vehicle to act unsafe around intersections, such as traveling straight through an intersection while in a turn-only lane, entering a stop sign-controlled intersection without coming to a complete stop, or proceeding into an intersection during a steady yellow traffic signal without due caution. In addition, the system may respond insufficiently to changes in posted speed limits or not adequately account for the driver's adjustment of the vehicle's speed to exceed posted speed limits.
FSD Beta software that allows a vehicle to exceed speed limits or travel through intersections in an unlawful or unpredictable manner increases the risk of a crash.

On September 23 2026 at night I was driving my car under FSD and am in full control of vehicle my car is on speed of 54-55mph and in the middle of the road without any obstacle present on the road the car did the super ghost breaking and my car stopped 100% with the screen turning red with a warning of collision w am thankful to god that no one is coming from behind otherwise it might be loss of lid or serious medical emergency . We are aware and keeping on checking the surroundings but it’s very hard or nearly impossible to go to 50 mph from 0 mph in 0 seconds.

On September 20, 2026, at approximately 10:26 AM EDT, I was driving my Tesla on ON I-76 E MM 332.4 with Full Self-Driving (Supervised) engaged. It was raining, the pavement was wet, and there was heavy road spray. The vehicle's front dashcam recording, time-stamped "Sep 20, 2026, 10:26:02 AM," shows the car traveling in the right lane at about 55-56 mph with the on-screen indicator reading "Self Driving." The vehicle then steered to the right, out of its lane, toward the concrete barrier and roadside embankment. The "Self Driving" indicator disappears about half a second before the vehicle leaves the lane, at about 54 mph. I did not steer or brake before the vehicle left the lane. The vehicle then lost stability, spun across the roadway, and slowed from about 53 mph to nearly a stop, facing against the direction of traffic. While facing oncoming traffic, it was struck by an oncoming white sedan, which sustained front-end damage my car had been declared a total loss. I believe the Full Self-Driving system failed to keep the vehicle in its lane in rain and gave too little or no warning before handing control back to me, leaving no reasonable time to recover. I have kept the original dashcam footage and have sent Tesla a written request to preserve and release all vehicle data for this event, including FSD engagement and disengagement logs, driver-input data, and stability control data. I can provide the footage and any records Tesla releases to NHTSA on request.

I was driving today in California, to TopGolf. I had to cross a railway crossing around [XXX] I think, or [XXX] - one of those 2, between [XXX] and [XXX] pm.. I was using Tesla's FSD driving to go on a local road. I was not distracted or occupied - the vehicle was going at a fairly high speed like maybe 35 or so, and in a couple of seconds, it broke the red and white safety gate at a railway crossing, inspite of it being down and the train being on the tracks - I WAS IN SHOCK. The car completely missed the red and white gate, and just blew past it, damaging my windshield completely to smithereens and throwing glass all over the car. I was in such SHOCK and TRAUMA that I literaly wanted to exit the car and sit down somewhere, but i gathered my composure and drove quickly to the nearest safest place I could stop, and got into some office parking garage. I then called the cops and reported it. This was such a shock, and thank the Almighty God that I wasnt hurt and that the train didn't just hit me. This HAS to be recalled, and there is no way this should be allowed on the roads. I used FSD a Lot and i will never use it again. An incident that occurred while Full Self-Driving (FSD) was engaged. During the drive, the vehicle unexpectedly steered toward the curb while making a right turn. I was actively monitoring the vehicle and immediately took control of the steering, but the vehicle still made contact with the curb. After the incident, the vehicle developed steering misalignment, vibration while driving, and abnormal noise, which suggests possible suspension or wheel damage. Since FSD was engaged at the time, I would appreciate it if Tesla could review the vehicle logs and investigate whether there was any system malfunction or unexpected behavior from the FSD system.

Comparison vehicle shows this is a defect, not normal wear. DESCRIPTION: Vehicle: 2023 Tesla Model 3 Long Range, VIN: [insert VIN], 9,816 miles Defect: Between September 2025 and November 2025, displayed range at 100% charge dropped from 345 miles to 300 miles—a 15% loss in just 2 months. Evidence This Is A Defect: I own a second 2023 Model 3 (base model) with 66,680 miles that only dropped from 272→260 miles (4.4% loss). The Long Range with 7x FEWER miles has 3x MORE degradation, indicating a battery pack defect. Service Refusal: Tesla Service ran “range analysis” but refuses to test battery capacity or check for dead cells. Only provided scripted responses about EPA vs real-world driving, which doesn’t explain why my high-mileage vehicle has healthier batteries. Safety Concern: Rapid cell degradation may indicate failing cells, thermal management defect, or fire risk. Unpredictable range loss creates stranding risk. Request: Investigate 2023 Model 3 LR battery packs for systemic defects. Require Tesla to perform cell-level diagnostics and replace defective packs. Documentation available upon request.

am reporting an issue involving unexplained cracking of my Tesla windshield with no known impact or debris strike. Last night at approximately 6:20 PM, while in Chapel Hill, NC, I entered my vehicle and noticed a crack on the driver’s side of the windshield. The outside temperature at that time was about 28°F. There was no audible rock strike and I have no recollection of anything hitting the glass. The morning low temperature was approximately 14°F. To my awareness, the passenger-side crack was not present earlier that morning. Approximately 15 minutes ([XXX]EDT) before writing this statement, I was seated in the passenger seat and noticed a second crack on the passenger side. The temperature had risen to about 34°F. Again, there was no strike event that I recall while driving or parked. Both cracks appeared within a short time span during an unusually cold period with temperature changes between 14°F and 34°F. The passenger crack visually resembles an impact point, but I cannot identify any incident that would have caused it. My concern is that these cracks may indicate thermal stress, structural flex, a material defect, or bonding/installation issue. Windshield failure without impact presents a safety hazard due to reduced visibility and potential compromise of structural integrity during a collision. There is NO mechanism to unlock the rear passenger doors in case of electrical failure. No Emergency mechanical door opening in case of emergency. Passengers will NOT be able to get out of the LOCKED vehicle during an emergency (i.e. crash, battery fire, submerged in water, etc). This is an EXTREMELY IMMINENT DANGER for small kids who are sitting in car seats, since it will take even longer to release them from their straps in order to get out. The small rear windows are useless for escape. THIS FLAW DESIGN HAS ALREADY CAUSED DEATHS. Changes SHOULD BE MADE to enable emergence mechanical opening of the rear doors.

Compliance arm bolt securing it to the subframe is fractured. Tesla has dismissed my ongoing concerns of vibration and instability while driving. Stating they performed an in depth inspection of all suspension and subframe components and found nothing of concern during last service appointment. I did my own inspection and found the fractured bolt. Sent photo to service and they are trying to play it off as normal and safe to drive. Contacted several manufacturers and mechanics stating otherwise. Told this will end in catastrophic failure and loss of vehicle control but apparently Tesla wants me to drive it to the service department for next service

While driving, the side mirrors closed and the doors locked by themselves. Then, the front trunk opened, the rear trunk opened, and when I opened the door, the headlights turned on and the front trunk opened again. When I opened the door another time, the side mirrors didn’t unfold, and the headlights started blinking. At high speeds (over 50 mph), I hear a ticking or metal-clanking sound, which is really scary. The car seems to be acting on its own, following some strange “manual” or behavior pattern. I feel unsafe driving it.

I was horrified to read of the deaths of three beautiful California girls in a Tesla Cyber Truck, that was involved in a crash, was on fire, and the electrical system malfunctioned resulting in them not being able to get out of the car. Tragically, they burned to death.(Recall too [XXX] ' incident as well as other incidents). I went to search the Internet for Model 3 (2023) to see if there were manual overrides in the event of crash/fire/electrical malfunction (even if related to dead battery) and found a You tube video (NOT information provided by Tesla mind you) that showed that on the front seats, there should be a lever not far from the door handle that you can pull to activate a manual release of the door. In the video, they said 'models' are different as to the rear seats but there 'should be' a lever. I could not find any manual override for the back seat passengers. So, I drove to the Tesla Dealer/Service department to have them show me how to activate the manual override for back seat passengers. The technician admitted that in many Teslas, including mine, THERE WAS NO SUCH CAPABILITY!! He admitted Tesla was acutely aware, they do not prioritize safety and that the back seat passengers would be stuck. He agreed that it will take a major class actions and major fines, and/or jury verdicts for them to do anything about it. Appalling and not acceptable. I would request that NHTSA mandate that all Teslas (probably models before 2025) lacking the device necessary to allow passengers to escape death due to failed electrical system be recalled to modify the car at Tesla's expense to comply with the reasonable expectations of consumers. This is a major design defect, unreasonably dangerous and presents a very real threat to the consumers who have no knowledge of the defect, much less where to look for the override (if it exists) because that information is not provided when you purchase the car. Due to the manual locks being looked at as a safety issue, I went and looked at the REAR door manual release for our Model 3. We also have a 2023 Model Y and I know they have a rear door release, as stupid as the location is, it is there. Well to my surprise, there is NO MANUAL REAR DOOR RELEASE in the model 3!!! How is this even safe? You're supposed to crawl through the front seats and go out the front door? Or even worse, fold the seats down and exit through the trunk? What happens if you're stuck in the back, can't get your seatbelt off? What if you are incapacitated?? What if you get rear ended and then pushed into the car in front of you and it makes it so the front doors are unable to be opened? (this happens quite often) What if the car rolls over, is upside down and you can't get to the front? There are a million situations that make a rear manual door release necessary. How does this even pass safety inspection??? How do you (as a department) even allow this to be acceptable. How are children supposed to do this? This has to be one of the most ridiculous things I've ever heard of and here I am, driving my kids every day around in it. They should be required to either retrofit it with a release that's accessible or replace the rear doors with an accessible release. I'm just in awe as to the failures here.

On June 4th I was in a hurry to get to work, pushed open my car door and started to slide in at which point the door unexpectedly to me swung back shut, clamping my head behind my ear pressing it against the top of the door frame. Everything stuck for about 1 second and then the door released my head. I am concerned that the door shouldn’t close so forcefully and should not bounce back closed when pushed open; that perhaps this contributed to the dangerous situation? There were other factors such as my car parking pad is slightly sloped. It seems this is an issue with the design of the car as opposed to a defect as there are no warning lights or recalls. I am still trying to figure out the effects of this injury. I have been to two doctors- not immediately because at first there were no immediate effects except for the discomfort of the pressure at the time. But less than 24 hours following the incident I developed a puffed up patch behind the ear where the door clamped, and now have developed a very swollen painless lymph node behind my other ear as well as headaches and some soreness that comes and goes. I have not contacted Tesla about the problem yet but am extremely cautious when I use the door and I don’t drive people in the car. I have included some photos but also just took some videos illustrating how the door swings back shut pretty forcefully when pushed open.

My pant leg got caught on the plastic seat trim under the seat. Kept my leg trapped so I fell out of the car on the ground in the parking lot. Happened again the same day, but I caught myself before I fell. The incident caused a hole and rip in my clothing, but the rest of my body went to the ground leaving my knee and other leg caught until the end. Had a different outfit on today and it caught again! I did not fall this time either. The time I did fall I hyperextended my wrist and landed on my hip and leg, creating bruising and a sore pelvis, knee and back. Went to massage therapist soon after. Any time I get into the car and immediately start driving before the car has time to catch up with what I am doing, the NHTSA mandated safety sounds played when the car is in reverse or drive does not play until the cars computer can catch up and begin playing the sounds. Most times this can take 3 seconds or so but today I had an incident where it took over 30 seconds before it would start playing. The same issue can be detected when restarting the infotainment screen by holding down both scroll wheels on the steering wheel. When doing this the car will cease playing the sound for around 5 seconds while the computer reboots.

While my car was in full self-driving mode, it attempted to make a lane change to the right which almost caused a crash into another vehicle. I had to manually take control of the car. On November 18, 2024, I reported this to the Tesla Service Center in Arlington, VA. Without even inspecting the car, Tesla concluded there was no safety concern and therefore, they declined my request to expeditiously inspect and repair the car. This vehicle has had several problems related to the autopilot system and other systems, and Tesla agreed in August 2024 to take back the vehicle after I filed a lawsuit under my state's lemon laws. I still have the vehicle as I'm still waiting for Tesla to provide instructions on how/when/where to return the car. I am documenting this incident to showcase all steps I took to remedy the defective product Tesla sold me in case the defect results in injury or damage. In the meantime, I have contacted me attorney and requested they send a demand letter to have Tesla make the repairs ASAP since there are clear and present dangers not yet resolved.

Side window spontaneously cracked. My car was parked and not being operated. I've watched video recorded by my car, and there was no one/thing around my car when the incident occurred.

The system failed to detect the driver having a seizure and had no way for the passenger to break the vehicle. People in the vehicle and Others were put at the risk of a seizing person accelerating in a car that could not be prevented by the car or by the passenger. No dealer/service center has confirmed the issues. The vehicle is currently being inspected by police. No known warning signs/messages before the incident.

Greetings, my name is --- My vehicle's VIN number is --- At the end of April 2024 I purchased a car from the seller, the seller claimed that my title was clean, he showed me pictures and it was indeed so When I came to Denver with this car, I put the car on the local registry and got a new title and it was clean too My wife was recently in an accident and the Tesla authorized service center didn't want to take the car in for service because the car has a salvage status in the title I started checking the car through several services that provide vehicle history, and found the following reports about it Source: [XXX] Date - [XXX] Type - Junk and Salvage Location - WESTCHESTER, IL Source: [XXX] Date - [XXX] Type - Total Loss Vehicle As we can see, this vehicle has 2 records of being Junk and Salvage and also a Total Loss vehicle But I was shown by the seller that the title is clean, also I was given a new title by the Arapahoe Motor Vehicle Services Office which is also clean. The official Tesla repair shops also refused to take my car for repair because they said that they do not support this title and that my car is salvaged. I would like to get the results of my car inspection from you if possible. I have also contacted the DMV of Texas and Colorado about this situation! On [XXX] around [XXX], my daughter drove her Tesla Model 3 on [XXX] from school. Two strange/scary accidents happened to two different Tesla cars (Model 3 and Y). It was raining with low/medium visibility. One of the cars was driven by my daughter, and luckily, both drivers were safe with very minor injuries. The cars had significant damage but were drivable, and there were no airbags out. These two accidents occurred within 3 to 5 minutes at the “SAME LOCATION.” When I asked them what happened, my daughter and other Model Y drivers described a sudden loss of control, with their cars spinning and becoming unresponsive to steering and braking. They then hit the guardrail on the center line. I checked the video recordings of our car multiple times; no vehicle-to-vehicle collision was involved. I also checked the road to see if there was any oil spillage, but the lane was clean. She didn't use Auto Pilot, FSD, and Cruse Control. The office recorded it as Hydro Plane in his report, but my concern was incidents that happened only to Teslas. Also, when I posted this incident on a Tesla WhatsApp group, multiple Tesla owners reported that they had a similar issue when it was raining. It seems like Sensors malfunctioning in the rain. Also, my daughter was driving below the posted speed (68/70), and she gave a written note to the officer too, but the officer recorded it as 70/70.
