I am filing an urgent complaint regarding multiple incidents involving my 2024 Tesla Model 3 (VIN: [XXX] ) that caused major safety risks, personal property loss, and serious disruption to my life. [XXX] – While in a left-turn lane with Autopilot engaged, the car attempted to go straight. When I took over, the system malfunctioned severely. (Photo evidence available) [XXX] – Remote support failed; car towed to Tesla Upland Service Center. June 27, 2025 – Issued loaner vehicle. July 1, 2025 – Tesla claimed I must pay for repairs. I requested a written evaluation report, but to date it has not been provided. July 20, 2025 – Without consent, Tesla remotely locked my vehicle with my personal belongings inside, violating my property rights. Aug 1, 2025 – Tesla promised review and report but refused to unlock my car. Aug 12, 2025 – After 10+ days, still no report. Upland “options” included threats that my locked car in my garage could be deemed stolen, causing severe mental distress. Demands: 1.Full investigation into Autopilot failure and handling delays. 2.Immediate delivery of written evaluation report. 3.Accountability for delays, threats, and property rights violations. 4.Compensation for all losses and disruptions caused. Contact: [XXX] I request a written response within 24 hours. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
Forward collision avoidance, Apr 21, 2025, crash
Vehicle: 2024 Tesla Model 3 Date of Incident: [Insert Date] Location: Intersection of [XXX] Operating Mode: Tesla Autopilot (Engaged) On the above date, while driving with Tesla Autopilot engaged, the vehicle initiated an unexpected and improper right turn at the intersection of [XXX] This maneuver was not indicated in the planned navigation route, nor was it consistent with the legal road markings or signage in the area. The vehicle turned into a clearly marked “Do Not Enter” and “Wrong Way” zone, without any driver input or confirmation. I attempted to intervene, but the turn happened abruptly and could not be safely overridden in time. This resulted in a collision, vehicle damage, and physical trauma to both occupants — including nausea and vomiting in my wife immediately after impact. Dashcam footage from the right repeater camera confirms the presence of multiple “Do Not Enter” signs visible at the point of the turn. We have requested the Tesla Event Data Recorder (EDR) logs to confirm that Autopilot was active and that steering input came from the system, not the driver. I am reporting this as a serious safety concern related to Tesla’s Autopilot system and its decision-making at intersections. If this had occurred in a busier traffic environment, the result could have been far worse. This type of navigational or perception failure should be investigated immediately. Both occupants are seeking medical treatment and continuing to deal with the emotional and physical aftereffects of this incident. Multiple instances of phantom braking, car will suddenly slam on brakes without any indication of danger or anything to avoid. This has happened at least a dozen times and we are fearful that it will cause an accident. Vehicle will also alert lane departure and attempt to automatically adjust, turn to another lane while in the appropriate lane. Multiple incidents, at least 30 in 6 months of ownership. No collisions as of yet, concerned there could be.

While driving from Costco in Fountain Valley to Best Buy in Costa Mesa, I was using FSD for the entire route. Everything was running smoothly until we approached [XXX] near [XXX] (Costa Mesa, CA). At that point, the FSD system incorrectly directed the vehicle to enter a freeway off-ramp, effectively driving into an [XXX] exit ramp in the wrong direction. This created a highly dangerous situation, as the ramp was intended for vehicles exiting the freeway, not entering. Fortunately, I was able to take over control of the vehicle and prevent a potential accident, but this was a very close call. Incident Details •Date: [XXX] •Time: Between [XXX] •Location: [XXX] (Costa Mesa, CA) •Route: Costco Fountain Valley → Best Buy Costa Mesa •Navigation Mode: Full Self-Driving (Beta) enabled throughout •Conditions: Daylight, dry roads, moderate traffic What Happened •FSD turned the car onto a road segment that functions as a freeway exit. •The vehicle entered the lane facing oncoming traffic exiting from [XXX]. •I immediately disengaged FSD and took manual control to correct the situation. Tesla has told me the AP4 computer has failed in my car which has resulted in them having to replace it. This issue impacts: emergency braking, blind spot detection, lane keep/centering, assisting with parking, automatic high beams and wipers, cruise control/ adaptive cruise control, Vehicles built in GPS to be stuck at one location every time, has made my touchscreen not automatically brighten or dim, my auto dimming mirrors be too dim in most driving conditions, constantly fail to install system improvements and safety updates, and because my vehicle keeps trying to do this, it has caused increased battery drain as well. This has increased the safety risk of my vehicle tremendously... Tesla has not seemed to publicly acknowledge the issue yet either.

The front camera that is known as Tesla vision consistently stays fogged up. This creates an issue when using auto-pilot that makes the car beep loudly and disengage auto-pilot putting others at risk if the car disengages and the driver is not quick enough to respond to the the disengagement. After several months of this and the problem does not go away after following Teslas advice per their user manual of pre conditioning the car and de fogging the windshield for a long period of time, I have scheduled a service appointment. The vehicle I own has not been inspected by Tesla, nor a third party service at the moment. There is no warning when the system will deactivate and I have had it happen several times on corners which is very concerning and scary when in the vehicle.

12/26/2024 around 5-6pm, Full Self Drive abruptly tried to accelerate into the intersection while sitting at a stop light. I stopped it immediately but was very surprised as FSD v13 has been exceedingly reliable and a pleasure to drive. This is the 2nd time this has happened after the holiday update of FSD 2024.45.25.5 (v13.2.2). Both times mine was the lead car at the light with no others ahead of me. This behavior has been reported multiple times in social media groups this past week by other FSD users. I entered a Tesla service request but have not heard back other than an automated appointment scheduled at the service center 8 weeks from my request and the incident.

the car consistently has phantom braking. the speed will drop around 15 miles of speed immediately. it also will slow on rolling hills and random long turns on cruise control. most recent date listed
Forward collision avoidance, Nov 19, 2024
Car randomly breaks or swerves when using cruise control. When there is nothing ahead of me have called tesla and been told it is called phantom breaking and that it is normal happens multiple times a day, almost causes collisions. Very dangerous.
Service brakes, Forward collision avoidance, Nov 2, 2024
About two weeks ago with driver set cruise control, the phantom braking was so severe that it took me down from 75 to around 15 if I remember correctly. The braking was so severe and at the same time self steered to the edge of the road along the guard rail that my body was unable to move quicker to correct the vehicle. Your system gets jolted violently without notice which takes a couple seconds to recover from and I decided to report this one after watching the San Francisco video. Fortunately, the two vehicles behind me were far enough away. This is absolutely not OK, I have had several severe braking incidents and each time it not only takes about 2-3 seconds to correct but also created back pain.
Vehicle speed control, Forward collision avoidance, Oct 27, 2024
Using cruise control to go with the flow of traffic the vehicle will slow down for any random flashing lights far away or vehicle in next lane causing all vehicles to slow down unnecessarily. FSD and beta programming does not belong in driver set cruise control.
Forward collision avoidance, Oct 23, 2024
While driving at highway speeds, the driver initiated what was to be a standard lane change from right to left to accommodate traffic merging onto the highway into the right-hand lane. About halfway through the lane change, the vehicle automatically and unexpectedly braked hard, locking the wheels. This led to high stress and back/neck pain. If traffic had not been light at the time, the vehicle would likely have been rear-ended. The vehicle was operating in traffic-aware cruise control during this event.
Vehicle speed control, Forward collision avoidance, Oct 21, 2024
While using Traffic Aware Cruise Control the vehicle would rapidly decelerate with no warnings of potential collision and with no actual emergency situation existing on the road. In a single trip that spanned both day and night there were three rapid declaration events occurring both when light or dark outside. Vehicle display wouldn't show any messages as to the reason for declaration.

Driver set cruise control has FSD imbedded that causes the vehicle to slow down drastically on its own. The car has brake tested several following vehicles so far. I’ve seen this before many times of other Teslas but thought it was the driver and now I know it was not. This causes a chain reaction of braking and smoking tires. I am positive this has created accidents but the ones involved have no idea how it started many vehicles ahead. The lane keep assist pillar cameras do not work in the dark, this has taken me by surprise already. The phantom braking on highways is awful, it is so bad that I am afraid to drive in the winter. The torque of the wheel sensor is not safe, when auto steer disables it can take you by surprise causing you to swerve into on coming traffic. The car accused me of using a cheat device one time so I decided to try it and surprisingly I found that it is actually much safer than using my body weight. These are extremely unsafe for busy traffic and slippery roads. I have videos showing it and will get more.

My wife and I recently purchased a new 2024 model 3 Tesla with the Autopilot and Full Self-Driving feature. We chose this vehicle because my wife has a long daily commute and we wanted to make the drive easier for her. We test drove a similar model in early August. When we were testing the Full Self-Driving feature, the car failed to safely exit the freeway three times in our short test drive, causing my wife to have to take over. Both times, the car seemed to become confused by traffic cones as it prepared to exit the freeway, causing it to either miss the exit or to change lanes rapidly and unexpectedly, nearly colliding with other vehicles. When we reported this to the employees at the Tesla dealership, we were told that the car needed a software update to address the issue. It is now a month later and we received our vehicle a couple of weeks ago. I have not heard of the release of any software update addressing the issue we experienced. Given that this is the exact feature that prompted us to purchase the car and our intention was to make things safer for my wife, not the other way around, I would very much like to find out when this will be addressed. There are many drivers who currently use the Full Self-Driving feature and I am concerned that it is extremely unsafe.
Vehicle speed control, Forward collision avoidance, Aug 28, 2024
There is a a very dangerous software issue with new Tesla models where there is a 1-2 second lag between when cruise control is disengaged and when it actually disengages. Especially, at high speeds this is dangerous because the car can continue maintaining acceleration for several hundred feet against driver controls. I’ve had to slam on the breaks several times because of this. This is not a glitch, and this is not unique to my tesla. It’s a known issue reported by the community that needs Tesla’s attention.

Twice, could not disengage Adaptive Cruise Control using right scroll wheel. Multiple times, haptic turn signals malfunctioning
Forward collision avoidance, Lane departure, Aug 15, 2024
The right scroll wheel used to turn on/off adaptive cruise and lane centering doesn't register inputs every time the button is pressed in. This makes turning the system on and off unpredictable and dangerous.
Forward collision avoidance, Aug 3, 2024
Several times while using Traffic Aware Cruise Control, the system will suddenly brake severely for apparently reason. At these times, I was on a lightly traveled Interstate with no vehicles ahead of me. Each time, it was during daylight hours and it was sunny. This has also occurred while using Autopilot with Autosteer and with Full Self Driving. Once, there was a vehicle close behind me, so this sudden braking can be dangerous.
Forward collision avoidance, Lane departure, Aug 3, 2024
While driving in the right lane of an Interstate using Full Self Driving (Supervised) mode (version 12.3.6), I was behind a motorcycle that was traveling slower than I. FSD was gaining on the motorcycle, but failed to move to the passing lane. I had to disengage the system so that the car didn't get too close to, or strike, the motorcycle. I manually moved to the passing lane and reengaged FSD. The car eventually decided to move to the right lane, behind the motorcycle. Again, it was gaining on the motorcycle and got too close, so I once again had to disengage FSD. It seems like the system didn't recognize the motorcycle or didn't recognize that it was traveling than slower than my vehicle.

Incident Description: One week after purchasing our new Tesla Model 3 2024 , we encountered several error messages upon returning to our car after a brief stop: "APP_w264 - Lane Departure Avoidance feature unavailable," "GTW_w075 - Steering assist reduced," and "UI_a020 - Steering assist reduced." With no cell signal available, we had to drive back slowly with hazard lights on. The steering was extremely tight, making it very difficult to maneuver. Tesla Roadside Assistance suggested towing the car and using an Uber coupon, which was impractical due to our location in the outskirts of Vancouver, Canada, with no Uber services, and our home in Seattle, USA.We drove over 100 miles to the nearest service centre in Vancouver, posing a significant safety risk on curvy freeway roads. The service centre confirmed that the wire supplying power to the steering was cut due to contact with the fan, and they repaired the issue. Conclusion: The steering failure of our Tesla Model 3 was a major safety concern. Driving with impaired steering could have led to a severe accident. The issue was due to a design flaw, with the wire contacting the fan. We request a thorough investigation to prevent future occurrences and consideration for the inconvenience and risk we faced. Component/System Failure: The wire supplying power to the steering mechanism was severed due to contact with the vehicle's fan. Safety Risk: The steering became extremely tight, making large turns nearly impossible and small turns very difficult. This posed a significant safety risk, especially while driving on curvy freeway roads. Inspection & Problem Confirmation: The problem was confirmed by the Tesla service centre in Vancouver. They verified that the wire was cut due to contact with the fan. Errors: "APP_w264 - Lane Departure Avoidance feature unavailable" "GTW_w075 - Steering assist reduced" "UI_a020 - Steering assist reduced"
