Recalls and investigations

  1. Structure, body, bumpersJul 7, 2026

Tesla, Inc. (Tesla) is recalling certain 2026 Model Y and 2024 Model 3 vehicles. The vehicles were originally built for a foreign market and do not have knee air bags, may not be equipped with certain interior safety countermeasures, and may not have been tested for bumper impact resistance procedures. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard numbers, 208, "Occupant Crash Protection," 201, "Occupant Protection in Interior Impact," and Part 581, "Bumper Standards."

A noncompliant vehicle may have reduced occupant protections, increasing the risk of injury during a crash.

Tesla will repurchase noncompliant vehicles currently registered in the United States, free of charge. Owner notification letters are expected to be mailed September 5, 2026. Owners may contact Tesla customer service at 1-877-798-3752 or resolutions@tesla.com. Tesla's number for this recall is SB-26-00-004.

  1. Electrical system, Structure, Other or unspecified, Sep 1, 2026, crash, fire

Client left the road and the vehicle hit a tree by the roadside. The vehicle caught fire and client was incapacitated and unable to operate the door handle from the inside or unlock the doors. Bystanders attempted to open all four doors from the outside, but they were all locked. Bystanders noted that the handles themselves were functional and believe it was an electrical issue keeping the doors locked. Bystanders broke the windows and attempted to open the doors from the inside, but they were unable to access the emergency door release because the driver door panel was on fire in proximity to the emergency release. Bystanders began mechanically disassembling the door to get him out and he was eventually extracted by the fire rescue team. The inability to open the door delayed our client being extricated from the vehicle and as a result he suffered severe injuries, including the loss of both legs, due to severe burns. The vehicle is currently being held by Copart Miami South and is available for inspection. So far, it has been inspected by the insurance company and our experts. Warning lamps, symptoms, etc.: UNKNOWN. Vehicle is badly damaged, so we are not able to recreate/confirm the problem at this time.

On XXX at approximately 12:56 PM Pacific on [XXX], my 2024 Tesla Model 3 VIN [XXX] was operating in Full Self-Driving (Supervised). The system detected oncoming traffic, stopped, then reversed to yield and backed into a parked 2015 Ford Transit van that had an occupant inside. I am the registered owner. [XXX] was the driver. TeslaCam saved events at 12:56:33 and 12:56:43. There was little visible damage, no injuries logged, and no police report. Tesla's consumer privacy form does not accept a crash narrative. I requested a Vehicle Data Report / copy of my data on XXX. USAA/[XXX] claim XXX later assigned 100 percent fault to the Tesla driver for backing, treating this as a conventional reverse collision rather than an FSD commanded maneuver. I am reporting this as a safety defect in FSD (Supervised) because the vehicle reversed into a parked occupied vehicle while the feature was engaged. Muffled or significantly quieter horn during rain. Users on Reddit generally agree that the sound typically returns to normal once the area dries out. Why This Happens: Speaker Design: Newer models (such as recent Tesla Model 3 and Tesla Model Y versions) utilize electronic pedestrian warning speakers or integrated units for the horn function rather than traditional enclosed metal horns. I have an issue with my car where every time I wash it or drive it even in light rain, the horn is extremely muffled. So muffled that I was sounding it driving down my road with people out walking, and no one heard it. I took it to the service center and they basically said “we confirmed your complaint, it’ll work when it dries out”. My car now has two notations on file from two visits to service center. This is a very wide spread problem - hundreds of comments online. Google: Tesla horn rain.

On June 16, 2026, while operating the "Autopilot" self-driving system, my vehicle autonomously activated the right turn signal to enter a parking space. Subsequently, the vehicle experienced an unexpected acceleration. Despite my immediate application of the brakes, the vehicle collided with a storefront window, resulting in property damage. My insurance provider covered all associated damages, including vehicle repairs. Immediately following the incident, I informed Tesla and requested access to the vehicle's electronic records, which were made available at a Tesla service center. Tesla is currently rescheduling my vehicle, acknowledging the severity of the self-driving system's malfunction. Concurrently, Tesla continues to charge me for the self-driving capability, despite having advised me to refrain from using it until all necessary vehicle tests have been completed. What further actions should I undertake, and how can the National Highway Traffic Safety Administration (NHTSA) provide assistance in this matter? Thank you.

Date: XXX, Time: 5.09.2026 State: [XXX] Location: [XXX] We were on Full Self Service Driving Mode A [XXX] came and hit my car in the passenger side behind the front wheel and front buffer. We were on left/fast moving lane and two semi-trucks were in the right lanes slight ahead of us (one had overweight sign). Something came out of the direction of trucks and hit our car with a big sound (moving object). When we stopped and checked the car later, it was evident that the damage was a sizable, it would have been something solid or heavy. Our concern is the Tesla was on Full Self Driving Mode, and it did not generate any alert, recording or more importantly, it did not break, change the driving direction nor try to avoid being hit by the object. This diminishes the reliability of Tesla FSD and raises serious safety concerns. We are in the process of filling insurance claim. COMPLAINT SUMMARY: CRITICAL SAFETY REGRESSIONS IN TESLA FSD SOFTWAREI am filing a formal safety complaint regarding severe, high-risk behavior regressions introduced across the last three consecutive builds of Tesla’s Full Self-Driving (Supervised) software. These defects create immediate collision hazards, cause major route disruptions, and require frequent, urgent driver intervention to prevent accidents.1. INTERSECTION AND TRAFFIC CONTROL VIOLATIONSRed Light Violations: The vehicle correctly identifies a red traffic light and comes to a full stop. However, after a brief pause, the software erroneously treats the steady red light like a stop sign and attempts to accelerate directly into active cross-traffic.Green Arrow Dead-Stops: When approaching a dedicated turn lane with an active, protected green turn arrow, the vehicle frequently applies aggressive braking and comes to a complete stop, creating an immediate rear-end collision hazard for drivers behind me.Phantom Braking: The vehicle aggressively brakes without warning for empty crosswalks, shadows, or distant roadside elements.2. ILLEGAL LANE NAVIGATION AND ROUTING ERRORSErroneous Turn Lane Diving: The vehicle frequently dives into dedicated left or right turn lanes even when the navigation route explicitly commands the vehicle to continue straight. This behavior forces improper turns, requiring backtracking and causing significant destination delays.Incorrect Lane Turning: The system regularly misses dedicated turn lanes entirely. It then attempts to execute sharp left or right turns directly from middle lanes or straight-only lanes, cutting off adjacent vehicles.Turn Signal Failures: The software initiates a turn signal to merge but then cancels the action, or conversely, attempts to execute a physical turn blindly after disabling the turn signal.3. RESTRICTED SPACE TARGETINGHandicap Parking Violations: Upon arriving at destinations, the automated parking software fails to recognize painted stencils and act

I am reporting a recurring safety defect regarding the exterior door handle assembly on my 2024 Tesla Model 3. On 6/15/26, the exterior door handle of caused a finger entrapment incident, resulting in an avulsion of a gel nail and significant trauma to the nail bed. The artificial nail remains trapped within the door assembly. Despite seeking service at the local Tesla Service Center, technicians failed to retrieve the object or address the entrapment hazard. This design creates a severe pinch-point hazard that poses an ongoing risk of injury to all users Despite bringing the vehicle in for service on 6/18/26 to have the hazardous object removed, the service center performed only a cursory visual inspection and refused to disassemble the handle, leaving the entrapment hazard in place. The design of the door handle creates a clear pinch-point/entrapment zone that poses a safety risk to all occupants. I am reporting this to document both the design defect and the failure of the manufacturer to address the resulting safety hazard upon request.

    - Tesla FSD - My son was driving on FSD on state highway, while on FSD, the car suddenly turned left and hit the concrete barrier and spun out of control. All the air-bags were deployed and car front crashed completely. The car is near totaled, My son was able to get out the car with a bruise on right hand. -No -No -No warning was on, the car was on full FSD and suddenly turned left and drove into the concrete barrier and crashed and spun , I had no time to react/take over.

I was using actual smart summon to get my vehicle out of my garage. While standing behind the vehicle, it indicated that the car was in drive with no reversing lights on. The vehicle then rapidly lurches backwards, striking me and a freezer. After reviewing the dashcam footage the vehicle stated it was in drive and never once activated its reverse lights, even when they was objects and a human behind the car, still reversed at a high rate of speed.

Attached is a photo of the lower driver-side knee bolster/under-dash panel after it detached while I was driving. The panel dropped directly into the pedal area and interfered with normal foot access to the brake and accelerator pedals. Given that no broken attachment points or damaged clips were reportedly identified, I am requesting a formal review of both the safety concern and the handling of this issue.

2024 Tesla model 3 driving on the turnpike heading south of NJ right between exit 5-6 when my vehicle suddenly and spontaneously spun out into the guardrail, while at the same time the automatic emergency brake was disabled. Weather conditions were slightly rainy, there was no signs of traction issues or alerts regarding any concerns. Luckily no other vehicles were hit. The vehicle has not been inspected by any other of the above questioned. No such warning signs, lamps, or symptoms prior to this failure. I do have the dash cam footage that shows the incident.

  12. Other or unspecified, Forward collision avoidance, Apr 20, 2026

On March 21, 2026, the vehicle’s Automatic Emergency Braking (AEB) and Traffic-Aware Cruise Control (TACC), suffered a severe "phantom braking" failure. While driving, the vehicle engaged in a violent, full-force emergency brake application without any external hazard or driver input. This unprompted deceleration caused the seatbelt to exert extreme pressure and trauma on my abdomen. As I was [XXX] pregnant, this abrupt stop was medically treated as a high-force accident. The trauma caused pregnancy abnormalities, requiring a 20-hour hospital admission for observation and fetal monitoring. I contacted Tesla customer service multiple times. Initially, I was told a service check was necessary, but the Service Center stated they could not provide assistance for "automatic behavior" issues. A corporate representative (Angel) admitted the vehicle should have been flagged for a formal medical claim and instructed me not to visit the service center. Despite this, I took the vehicle to the Service Center on April 15, 2026. Technicians confirmed they had no findings or solutions regarding the phantom braking event. No warning lamps, audible alerts, or system messages appeared prior to the failure. Upon requesting the vehicle data logs for the date of the incident, I noticed that the specific timeframe of the event was "missing" from the record. The manufacturer cited a VIN-matching error as the cause for the data gap. On April 20, 2026, Tesla corporate denied that a claim had ever been filed and stated they would not take responsibility for injuries related to pregnancy or a fetus, claiming these do not meet their internal threshold for medical injury. The vehicle remains available for inspection. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

  13. Steering, Other or unspecified, Lane departure, Jan 20, 2026, crash

I purchased a 2024 Tesla Model 3 a little over a year ago. Recently, and after an update, myself, and other drivers in my life, noticed the self-driving feature was malfunctioning - to the point I used the audio reporting feature to report this problem twice to Tesla. The car would veer off the road when flasing lights appeared and would jerk suddenly for no reason when it thought there was a obstruction in the road (sticks, plastic bags, etc.). Last month this malfunction has caused an accident, and my car has been totaled, my insurance will not cover it, and I have been advised by two separate attorneys to sue. This has been litigated in court with lawsuits against Tesla (See Benavides v. Tesla; see also IN RE: TESLA ADVANCED DRIVER ASSISTANCE SYSTEMS LITIGATION). I had notified Telsa twice of this issue, the car left the lane and did not notice a concrete barrier on the left side going north on I-95 in or around Broward County. To my knowledge no one has inspected the car. I was not warned that this car would cause an accident, the problem first appeared when I reported the issue using the voice reporting feature around November 2025. It is currently located at a tow facility in Broward County, FL and can be inspected there.

Tesla Model 3 Software & Trunk Security Issue Vehicle: Tesla Model 3, 2024 Issue Date: September 25, 2025 I am reporting a serious software reliability and security defect in my Tesla Model 3. On September 25, 2025, the trunk opened on its own overnight due to what appears to be a software malfunction, resulting in the theft of $500 in personal belongings. The Tesla app sent only one notification while I was sleeping, with no alerts for over eight hours, failing to provide proper security warnings. Tesla customer support was unable to resolve the issue and suggested multiple drivers might have access, even though only one friend has access to the car and it was not her doing. Vehicle data from Tesla privacy support showed two hours missing during the trunk event, and Tesla could not explain why, only speculating a software glitch. This incident demonstrates a critical software and security failure that compromises vehicle safety and personal property. Tesla has failed to provide an explanation, solution, or preventive fix. I am reporting this as a serious software reliability and consumer safety concern. Best, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

  16. Air bags, Seat belts, Other or unspecified, Nov 25, 2025

Safety restraint warning in car, unsure of actual issue but warning keeps popping up. Presumed that the seatbelts or airbags would not properly work in event of a crash. Dealer/company says it's a known issue, with other cars needing a fix too, however they aren't able to specify exactly the issue or issue a timeframe on repair. Issue first started appearing in Late August of this year, and has become more frequent as of recent. Potentially this is a critical safety issue regarding airbag/seatbelt's potentially failing to work as designed for all similiar models.

  17. Other or unspecified, Nov 3, 2025

Turn signal indicator buttons stick and/or activate the opposite turn indicator of the button pressed. Issue is persistent and I have had two other steering wheels replaced and all three have the same problem. Tesla technician told me that it is an issue with the steering wheel and there was no fix. Have a video of the problem but I am currently unable to upload.

  18. Other or unspecified, Oct 17, 2025

I have reported to Tesla how dangerous it can be long term to have one pedal driving and to use the accelerator as a cancelation mode to it’s many errors, see uploaded example picture. Also, having lighted notifications on mirrors causes people to subconsciously ignore actual blinkers of other vehicles; the only thing that should be on a side mirror is an adjustable fish eye mirror that allows to see the entire side lanes and tire placement.

  19. Other or unspecified, Oct 5, 2025

The turning signal buttons (this car has buttons instead of a typical stalk) often becomes stuck. It’s like all of the buttons on the left side of the steering wheel are already pushed down, and therefore, I cannot indicate whether I’m turning by pushing that turning button.

  20. Other or unspecified, Aug 21, 2025, crash

Button turn signal on New Model 3 is vet dangerous. When steering is rotated, is is impossible to find the Signal Button because of the location of the Button is also rotated according to the Rotation of the Steering Wheel. It caused me a traffic accident when I tried to find the Signal Button when there was traffic congestion in the middle os traffics. This must be replaced traditional turn signal lever.

  21. Other or unspecified, Aug 14, 2025, crash

While using Tesla’s Auto Park feature, the vehicle struck an obstacle without detecting it before or after impact. The system failed to log a collision event in its detection systems, giving no warning and providing no reasonable opportunity for driver intervention. Data retrieved directly from Tesla confirms that forward/side collision sensors did not activate, “Near Deploy” collision detectors remained inactive, and although a “Crash Algorithm Wake-Up” was triggered, the vehicle did not brake or attempt avoidance. This represents a potential systemic defect in Tesla’s Auto Park detection logic and/or related hardware that could result in collisions without driver awareness or system logging — a safety risk for any Tesla driver using Auto Park.

  22. Other or unspecified, Jul 14, 2025

Newer Tesla Model 3s (2024+ Hihgland) removed the physical turn signal and drive shift stalks. This creates serious safety concerns: 1. Inconsistent or Difficult/Impossible Signaling: In certain turning maneuvers, especially while driving with a sharp wheel angle, such as on curves, roundabouts, or a turn immediately following another, it becomes physically difficult or impossible to reach and activate the appropriate touch signal. This failure to clearly signal intentions to other drivers increases the likelihood of collisions and endangers both the driver and others on the road. 2. More Frequent Signaling Errors: The touch buttons for left and right turn signals are placed side-by-side, with no tactile distinction. This can lead to frequent accidental activation of the wrong signal, especially in high-pressure or fast-response situations. Misleading or absent signals confuse surrounding drivers and pedestrians, increasing the risk of sideswipes, intersection crashes, or pedestrian strikes. 3. Delayed or Failed Emergency Shifting: The removal of a physical gear shift stalk in favor of an on-screen interface causes dangerous delays when quick, instinctive gear changes are needed, such as switching from Drive to Reverse to avoid a crash or recover vehicle control. Because there is no tactile control, blind muscle-memory responses are difficult or impossible, and looking away from the road to the display may be necessary, increasing the likelihood of a collision in time-critical situations where every second counts. Tesla has since reintroduced turn signal stalks in the updated Model Y (Juniper), and older Model 3s had them, proving this is a design choice, not a limitation. These changes reduce the ability to safely operate the vehicle under pressure and should be reviewed for compliance with safety standards.

  23. Other or unspecified, Jun 11, 2025

When driving in the rain or in wet conditions, the car’s horn cannot be heard at all. It is extremely muffled and can pose a serious hazard to the drivers if they cannot be heard.

  24. Other or unspecified, May 31, 2025

The horn is extremely quiet especially after driving in puddles or in the rain. The horn being a speaker on the front of the vehicle causes it to become clogged and practically muted when used in the rain or after driving through water. The speaker horn is extremely quiet and unsafe.

TURN SIGNAL BUTTONS JAMS OCCASIONALLY. VERY DIFFICULT TO REPRODUCE DUE TO THE INFREQUENT NATURE OF THIS ISSUE, BUT IT HAPPENS ABOUT TWICE A WEEK IN DAILY DRIVING. SEVERAL OWNERS OF THIS MODEL VEHICLE HAVE REPORTED THE SAME ISSUE ON THE FORUMS. DEALER/MANUFACTURER REFUSES TO ADDRESS THIS ISSUE OTHER THAN TO SAY THEY KNOW THIS IS A KNOW ISSUE. IN SEVERAL INSTANCES, THE TURN SIGNAL BUTTON JAMS AT VERY CRITICAL TURNS ON THE HIGHWAY WHICH CREATED A SAFETY HAZZARD. I HAVE SEVERAL VIDEO CLIPS OF THIS OCCURRENCE.
