As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 108, "Lamps, Reflective Devices, and Associated Equipment."
Headlight low beams that are too bright can reduce visibility for oncoming drivers, increasing the risk of a crash.

As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 138, "Tire Pressure Monitoring Systems."

(Tesla) is recalling certain 2016-2023 Model S, Model X, 2017-2023 Model 3, and 2020-2023 Model Y vehicles equipped with Full Self-Driving Beta (FSD Beta) software or pending installation. The FSD Beta system may allow the vehicle to act unsafe around intersections, such as traveling straight through an intersection while in a turn-only lane, entering a stop sign-controlled intersection without coming to a complete stop, or proceeding into an intersection during a steady yellow traffic signal without due caution. In addition, the system may respond insufficiently to changes in posted speed limits or not adequately account for the driver's adjustment of the vehicle's speed to exceed posted speed limits.
FSD Beta software that allows a vehicle to exceed speed limits or travel through intersections in an unlawful or unpredictable manner increases the risk of a crash.

While freeway driving with autopilot (adaptive cruise control) engaged the car will suddenly break VERY hard when there is no reason to. This is SUPER dangerous when traveling at 70 mph and someone is behind me. This is a known issue (phantom breaking) that has been a problem for years and not resolved.

The contact owns a 2019 Tesla Model 3. The contact stated while driving 65 MPH, the vehicle experienced "phantom braking". The contact stated no warning light was illuminated. The contact was able to continue driving to his destination. The contact informed the manufacturer of the failure through the Tesla App and the mobile service unit was dispatched to inspect the vehicle. The vehicle was not repaired. The contact stated that recently he had experienced the failure five times within an hour and a half. The manufacturer had been informed of the failure. The failure mileage was approximately 21,100.

The contact owns a 2019 Tesla Model 3. The contact stated that while driving at 60 MPH, the vehicle experienced phantom braking while there was no other vehicles or objects around. There was no warning light illuminated. The contact depressed the accelerator pedal and was able to continue driving. The contact stated that the failure had been reoccurring increasingly while driving. The contact stated that the vehicle was not designed with self-driving mode. The manufacturer was notified of the failure but provided no assistance. The vehicle was not diagnosed or repaired. The failure mileage was approximately 50,500.

The contact stated while driving approximately 70 MPH with adaptive cruise control mode and passing a semi-truck, the vehicle experienced phantom braking and pulled hard to the left on three occasions. There were no warning lights illuminated. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure and advised the contact to provide a two minute time frame of the next occurrence of the failure. The failure mileage was approximately 27,019.

On a recent cross-country trip in our Tesla, we experienced "phantom braking" a number of times when we had the adaptive cruise control engaged. We were not using more advanced features on any of these occasions.

While the vehicle has adaptive cruise control engaged, there are times it applies braking (sometimes aggressive braking) for no reason (commonly referred to as phantom braking). It’s almost as if the vehicle thinks there is an object in front of it and is applying emergency braking, however there is nothing on the road. In my experiences, it happens mostly on the Highway at speeds 60-80mph. This happens on clear days, with no weather concerns. This is a severe issue that will cause accidents (if people are following closely behind me, there is a concern of them hitting me). It is scary and concerning ; this needs to be immediately addressed and corrected.

I was traveling approximately 70 mph in passing lane when I engaged (standard) autopilot, as I approached to pass a vehicle with another vehicle following me in the same lane as I was traveling. My vehicle (phantom) braked hard to which the wheels began to squeal slightly and the vehicle behind me had to brake hard to avoid hitting my vehicle. I almost immediately applied pressure to the accelerator pedal to avoid a complete stopping of the vehicle and to which the vehicle responded to accelerate. And I disengaged the autopilot immediately by applying an upward motion on the gear stock.

I experienced more than 10 incidents of “phantom braking” while on Interstate highways (I90 and I29) on a round trip from Solon Iowa to western South Dakota. ( late Sep 2022). There were no apparent obstructions, nearby vehicles, or other dangers. In several incidents I was traveling at the speed limit (80 mph) and the car braked so suddenly and hard it jolted wife and terrified her. If a car or truck was behind me I would have almost certainly had a serious accident. This behavior seems to have gotten much worse in the past few months, although I have seen it occasionally over the 3 years (48000 miles) I have owned the car. I have noticed it happens most often near the crest of hills.

I have experience phantom braking on my Model 3 many times when I used FSD(full self drive) on the freeway. The car just randomly slows down / decelerates abruptly even if there are no obstacles or other cars in front. This results in a serious situation when the vehicle behind may ram into you causing a fatal accident. I have always reported this issue using voice control REPORT BUG and got acknowledgement saying THANKS FOR THE FEEDBACK. But I have never heard from Tesla that they have fixed or are fixing it. This issue is happening almost every time I used FSD on freeway even today (Aug 30, 2022)

My friend was driving the vehicle (a 2019 Tesla Model 3) while it was engaged in Full Self-Driving Beta 10.12.2 mode. The vehicle made a turn left and found itself in a lane that was coned off. Traffic was queuing in the adjacent lane. The vehicle edged forward into the closed lane and waited to join the lane to the right. Even though other drivers were giving way, the vehicle failed to move. The driver took manual control in order to complete the manoeuvre. My friend was driving the vehicle (a 2019 Tesla Model 3) while it was engaged in Full Self-Driving Beta 10.12.2 mode. The vehicle approached a busy intersection to turn left. Although drivers were giving way to the vehicle, it failed to complete the turn in a timely manner. The vehicle edged forward and then rapidly made the turn at 12mph . My friend was driving the vehicle (a 2019 Tesla Model 3) while it was engaged in Full Self-Driving Beta 10.12.2 mode. The vehicle stopped short of a STOP sign at a junction then slowly moved to the middle of the junction, failing to make a right turn. After manual re-routing the vehicle brakes for no reason other than a pedestrian is close by. The vehicle then fails to make a left turn successfully, cutting the corner. My friend was driving the vehicle (a 2019 Tesla Model 3) while it was engaged in Full Self-Driving Beta 10.12.2 mode. After successfully crossing a junction the vehicle accelerated too quickly on a narrow residential road with another vehicle approaching from the opposite direction. My friend was driving the vehicle (a 2019 Tesla Model 3) while it was engaged in Full Self-Driving Beta 10.12.2 mode. Cruising at 30+ mph the vehicle was late to slow down ahead of a left turn junction. The vehicle then cut across the junction as it was moving too fast to make the turn correctly. Yesterday I had the most frightening driving experience of my entire life. It was raining heavily and I was going about 35 mph on the local road in my model 3 standard, 3 years old. Suddenly I lost all traction and was slipping side to side +/-90 degrees from the direction of traffic. I immediately took my foot off the accelerator and tried to slow down but didn’t brake. I swung 180 degrees each way about 5 times. Luckily no one hit me but I was taking up all lanes as I careened all over the road. I can’t believe how easily the model 3 loses traction in the rain. This is unacceptable.

The vehicle failed to recognise a speed limit change approaching a sharp left turn. The vehicle took the turn too fast at 29mph veered into the oncoming lane. My friend was driving the vehicle (a 2019 Tesla Model 3) while it was engaged in Full Self-Driving Beta 10.12.2 mode. At approx 20 mph the vehicle entered a stretch of road works with temporary signage and cones. The vehicle performed an emergency stop for no reason to the frustration of other road users. At 10mph the vehicle attempted to change lanes whilst there was another vehicle in the other lane. If the driver had not intervened there may have been a collision or the other vehicle would have to brake hard to avoid contact. The vehicle was navigating through roadworks with temporary signage and cones in place. This seemed to interfere with the FSD with the vehicle moving left to right in an erratic manner and accelerating to 28mph whilst still in the roadworks zone.
