    Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2006-2011 Toyota RAV4 vehicles manufactured October 31, 2005, to September 7, 2010, and 2010 Lexus HS250h vehicles manufactured July 6, 2009, to August 26, 2010. The affected vehicles have rear suspension arms (rear tie rods) with adjusting lock nuts that may have been improperly tightened after servicing. As a result of being loose, the arm may have thread damage and may rust, possibly leading to the failure of the arm and an abrupt change in the vehicle's alignment.

    Failure of the rear tie rod could cause a loss of vehicle control, increasing the risk of a crash.

    Toyota will notify owners and dealers will replace both rear suspension arms and encapsulate the locknuts, free of charge. The recall began on September 30, 2016. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's numbers for this recall are G0V for Toyota vehicles and GLK for Lexus vehicles. Note: This recall supersedes recall 13V-383, in which some vehicles were only inspected, sealed, and did not have both arms replaced. The affected vehicles were previously recalled under 13V-383, but did not have both rear suspension arms replaced at that time. Those vehicles need to have both arms replaced as part of this recall remedy.

    Southeast Toyota Distributors, LLC (SET) is recalling certain 2005-2011 4Runner, Highlander, Sequoia, and Sienna, 2005-2010 Avalon and Tundra, 2006-2008 Camry Solara, 2006-2010 Highlander hybrid, 2006-2011 Rav4 and Tacoma, 2007-2010 FJ Cruiser, 2007-2011 Camry hybrid, 2009-2010 Venza, 2010-2012 Prius, 1988-1990 and 2005-2010 Camry, and 1989 and 2009-2010 Corolla vehicles equipped with aftermarket accessory seat heaters with a copper strand heating element. The electrical wiring in the seat heaters may be damaged when the seat cushion is compressed.

    If damaged, the copper strand heating element may short circuit, increasing the risk of a fire.

    SET will notify owners, and dealers will disconnect the seat heaters, free of charge, and refund the purchase price of the seat heater accessory. The recall began on July 14, 2016. Owners may contact SET customer service at 1-866-405-4226. SET number for this recall is SET16B.

    Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2006-2012 RAV4 vehicles manufactured July 28, 2005, to December 19, 2012, and 2012-2014 RAV4 EV vehicles manufactured July 24, 2012, to August 29, 2014. In the event of a frontal collision, the rear seat belts for the outboard seats may be cut by the seat cushion frame.

    A cut seat belt may not properly restrain the seat occupant, increasing their risk of injury in a crash.

    Toyota will notify owners, and dealers will install a protector over both rear seat cushion frames, free of charge. The recall began on June 24, 2016. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is G0F.

    Toyota Motor Company (Toyota) is recalling certain model year 2009-2011 Tundra, Sequoia, Corolla, Corolla Matrix and Scion xB, 2008-2011 Highlander and Highlander Hybrid, 2007 Camry and Camry Hybrid, 2009 Camry and Camry Hybrid, 2006-2011 RAV4, 2006-2010 Yaris, and 2009-2010 Scion xD and Pontiac Vibe vehicles. During the manufacturing of the Power Window Master Switch (PWMS), grease lubricant may have been inconsistently applied to the sliding electrical contacts.

    If the sliding electrical contacts are not protected by lubricant, debris and moisture that get into the switch may cause a short circuit and the switch assembly may overheat and melt, increasing the risk of a fire.

    Toyota will notify owners, and dealers will inspect the switch, applying a lubricant if no abnormality is found. If abnormality is found, the PWMS circuit board will be replaced, free of charge.     Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2009-2012 RAV4 vehicles manufactured October 27, 2008, to December 19, 2012 and 2012-2014 RAV4 EV vehicles manufactured July 24, 2012, to August 29, 2014. In the affected vehicles, water may drip onto the joint that connects the wiper linkage to the wiper motor.

    Over time, the dripping water may cause corrosion and wear at the joint, potentially resulting in the separation of the wiper linkage. Once this occurs, the wipers would not function, reducing driver visibility and increasing the risk of a crash.

    Toyota will notify owners, and dealers will replace the water diverter channel. In addition, a new wiper linkage with a wiper motor crank arm will be installed. The wiper linkage will have a protective cover over the wiper linkage joint to minimize water splash and a retainer ring to prevent the wiper arm from separating from the wiper motor.     Southeast Toyota Distributors, LLC (SET) is recalling certain model year 2010-2011 Prius and Corolla, 2009-2011 Venza, 2006-2010 Avalon, 2007-2010 FJ Cruiser, 2005-2011 Sienna and Sequoia, 2006-2011 Tacoma 4x2 and 4x4, Camry, Highlander, Tundra 4x2 and 4x4, and 2007-2011 Rav4 and 4Runner. The affected vehicles may experience compression of the seat cushion which may damage the seat heater wiring.

    Damage to the seat heater wiring could cause the wires to short, increasing the risk of the seat burning and causing personal injury to the occupant.

    SET will notify owners, and dealers will disconnect the heaters with copper strand heating elements and refund the purchase price of the seat heater accessory, free of charge. The recall is expected to began in January 2015. Owners may contact SET customer service at 1-866-405-4226.

    Toyota is recalling certain model year 2006 through 2011 RAV4 and 2010 Lexus HS250h passenger vehicles manufactured from October 2005 through September 2010. If the nuts for adjusting the rear wheel alignment are improperly tightened during service, the rear suspension arm (rear tie rod) may develop unwanted movement and rust leading to thread damage and eventual failure. Failure of the rear tie rod will cause an abrupt change in the vehicle's alignment.

    Failure of the rear tie rod could cause a loss of vehicle control, increasing the risk of a crash.

    Toyota will notify owners, and dealers will inspect the tie rods and replace any that have rust or corrosion on the alignment adjustment threads. All of the rear tie rods will then be encased in an epoxy to prevent future environmental damage. Due to limitations in parts, the recall remedy will be launched in phases from September 2013 through mid-April 2014. All owners will receive notification of the recall, with most receiving second notifications when parts are available in their area. IMPORTANT: This recall supersedes Toyota and Lexus recall 12V-373. All vehicles that have been inspected and remedied under the previous recall need to be re-inspected and have the appropriate remedy applied.

    Southeast Toyota is recalling certain model year 2008 and 2010-2013 Toyota Tundra, 2010-2012 Rav4, 2012 Toyota Sequoia, 2010-2011 Toyota Corolla, 2010-2011 Toyota Camry and Camry Hybrid, 2010-2013 Toyota Highlander and Highlander Hybrid, 2010-2013 Toyota FJ Cruiser, 2011 Toyota Land Cruiser, 2010-2013 Toyota Venza, 2010-2011 Toyota 4Runner, 2010-2013 Toyota Tacoma, 2011-2012 Toyota Sienna, 2012 Toyota Prius, 2013 Scion FR-S, 2011 Scion XD, 2011 Scion XB, and 2012 Scion TC vehicles. These vehicles were sold with labels that were outside the allowable one percent of accuracy of actual weight added. Thus, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) Number 110, "Tire Selection and Rims."

    An inaccurate label could lead to owners overloading their vehicles and tires. An overloaded vehicle can result in a tire failure which may result in a vehicle crash, personal injury, or property damage.

    Southeast Toyota will notify owners and provide a corrected label with instructions concerning its installation.     Southeast Toyota Distributors, LLC (SET) is recalling certain models interspersed through model years 2009 through 2013 as follows: model year 2009-2012 Tacoma, 4Runner, Camry, Camry Hybrid, Prius, and RAV4; model year 2009-2010 Avalon, FJ Cruiser, and Highlander Hybrid; model year 2010-2013 model year Corolla, Sienna and Tundra; model year 2009-2013 Highlander and Venza; model year 2012 Prius V; and model year 2010-2012 Sequoia. During modification by SET to include accessories such as leather seat covers, seat heaters or headrest DVD systems, these vehicles may not have had the passenger seat occupant sensing system calibration tested. Without passing the calibration test, the occupant sensing system may not operate as designed.

    If the front passenger seat occupant sensing system is out of calibration, the front passenger airbags may not deploy or they may deploy inappropriately for the passenger's size and position. This could increase the risk of personal injury during the event of a vehicle crash necessitating airbag deployment.

    Southeast Toyota will notify owners, and dealers will test the sensitivity of the occupant detection sensors, and recalibrate them as necessary. The recall began on March 21, 2013. Owners may contact Southeast Toyota at 1-800-301-6859.

IN THE CURTAIN SHIELD AIRBAGS, PROPELLANT WITH THE INCORRECT SPECIFICATION WAS USED FOR THE INITIATOR ASSEMBLED IN THE AIRBAG INFLATOR. IN THIS CONDITION, THE INFLATOR MAY NOT PERFORM PROPERLY, CAUSING ONE OR BOTH CURTAIN SHIELD AIRBAGS TO NOT DEPLOY.

    IN THE EVENT OF A CRASH, THE AIRBAGS MAY NOT DEPLOY AND WILL NOT BE ABLE TO PROPERLY PROTECT AN OCCUPANT, INCREASING THE RISK OF INJURIES.

    I NOTICED THAT THE STEERING WHEEL FELT A BIT "LOOSE" AT ANY RATE OF SPEED. I ALSO OBSERVED THAT MY R BACK SEAT SHAKES WHEN DRIVING AT HIGH RATES OF SPEED(50 MPH +). MY VEHICLE HAS JUST OVER 66K MILES ON IT. I MAINTAIN IT ACCORDING TO THE USER MANUAL. I BROUGHT MY CAR IN A FOR FOUR WHEEL ALIGNMENT. THE REAR TIRES COULD NOT BE ALIGNED BECAUSE THE NUTS FOR ADJUSTING THE REAR WHEEL ALIGNMENT WERE FROZEN AND COULD NOT BE REMOVED. IN ORDER FOR THE CAR TO BE ALIGNED, THE REAR TIRE ROD NEEDS TO BE REPLACED. THERE WAS A RECALL ON THIS BACK IN AUGUST 2013 ( FOR 2006-2011 RAV4), HOWEVER, MY PARTICULAR VIN NUMBER DOES NOT QUALIFY FOR THE RECALL THUS I HAVE TO PAY OUT OF POCKET IN ORDER TO FOR MY CAR TO BE ALIGNED CORRECTLY (REPLACED THE REAR ROD).     TL* THE CONTACT OWNS A 2011 TOYOTA RAV4. WHILE PARKING THE VEHICLE AND DEPRESSING THE BRAKE PEDAL, THE VEHICLE SURGED FORWARD INDEPENDENTLY. THE VEHICLE CRASHED INTO THE FRONT OF ANOTHER VEHICLE. THE FAILURE WAS PERSISTENT. A POLICE REPORT WAS NOT FILED AND NO INJURIES WERE SUSTAINED.     TODAY WHEN I WAS PARKING MY CAR IN PARKING LOT, I REALIZED THAT IT IS VERY DIFFICULT TO MOVE MY STEERING WHEEL TO LEFT AND RIGHT. IT WAS REALLY DIFFICULT OR REQUIRED A LOT OF FORCE TO MOVE MY STEERING WHEEL WHILE PARKING. THE VEHICLE WAS IN STATIONARY OR MOVING VERY SLOWLY AND I WAS NOT ABLE TO MOVE STEERING WHEEL IN EITHER DIRECTION EASILY. THIS IS THE FIRST TIME I HAVE FACED THIS STEERING WHEEL ISSUE I HAD NEVER FACED THIS ISSUE BEFORE WHILE DRIVING MY VEHICLE.

    AT A ROUTINE OIL CHANGE WAS TOLD THERE WAS A "TRANSAXLE LEAK" THAT REQUIRED IMMEDIATE ATTENTION FROM AN AUTHORIZED DEALER. THERE WAS NO VISIBLE LEAK IN THE DRIVEWAY, NO CHECK ENGINE LIGHT OR OTHER WARNING. SO WHEN THIS DEVELOPED IS UNKNOWN. TOLD NOT SAFE TO DRIVE. REPAIR WOULD BE $3000. TRANSMISSION AND DIFFERENTIAL HAD TO BE REMOVED TO SEAL LEAKING. THIS VEHICLE HAS 34K MILES ON IT. WAS TOLD THIS ONLY SEEN IN CARS OVER 100K. HAS BEEN WELL MAINTAINED--ALL SERVICE CHECKS AND RECALLS COMPLETED. TOYOTA ACCEPTS NO RESPONSIBILITY. NO SIGN OF A POTENTIALLY DANGEROUS SITUATION.

    SUSPENSION: WHILE DRIVING IN THE INTERSTATE, I HEARD A "POP" AND THE SOUND OF SOMETHING SLIDING FROM THE BACK PASSENGER SIDE TOO THE BACK DRIVER SIDE OF THE VEHICLE. I THEN FELT THE BACK OF THE VEHICLE "WOBBLING" AND I HEARD A "CLUCKING AND RUBBING" SOUND. I MANAGED TO GET TO A REPAIR STATION, AND OBSERVED THE BACK DRIVER'S SIDE TIRE LEANING INWARD AT THE TOP, AND IT HAD BEEN RUBBING AGAINST THE WHEEL WELL AND A CORD. THE REPAIRMAN LOOKED IT OVER AND FOUND NO REASON FOR A BOLT THAT RANGED ABOUT 5-7 INCHES TO SHOOT OUT, RESULTING IN THE BACK WHEEL TO ALMOST FALL COMPLETELY OFF THAT WAY, AS THE UNDERCARRIAGE "LOOKED PRETTY GOOD." I JUST PURCHASED THE VEHICLE JUST OVER 3 MONTHS AGO FROM A CERTIFIED DEALER. THE CARFAX REPORTED NO PREVIOUS ACCIDENTS AND THE CAR WAS REPORTEDLY MAINTENANCE AS RECOMMENDED, (EVERY 3000 MILES). I'M AFRAID TO DRIVE IT NOW. WHAT'S TO KEEP THIS FROM HAPPENING TO THE OTHER THREE TIRES???

THE PARTS HAD NOT BEEN SUPPLIED BY THE MANUFACTURER TO THE DEALER FOR OVER A YEAR. THE DEALER KEPT STATING THAT THEY WOULD CALL, BUT THE CONTACT HAD NOT RECEIVED A RESPONSE.     CAR SITTING IN PARKING LOT. STARTED ENGINE IN PARK, ENGINE SUDDENLY ACCELERATED HAD TO TURN IGNITION KEY OFF TO STOP ENGINE FROM RUNNING AT HIGH SPEED. WHEN RESTARTED ENGINE RAN AT NORMAL IDLE SPEED. CHECKED FLOOR MAT AND HOSES ON ENGINE, ALL OK. SCHEDULED APPOINTMENT WITH TOYOTA DEALER FOR 3/6/2017. CAR HAS 28,563 MILES ON ODOMETER AND I AM THE ORIGINAL OWNER. CAR IS GARAGED, NO ACCIDENTS AND IS REGULARLY SERVICED BY TOYOTA DEALER. OUTSIDE TEMPERATURE OUTSIDE WAS 41 DEGREES. I CHECKED THE FOOT PEDAL (THROTTLE) FOR FREE TRAVEL IT WAS OK.

    AT APPROXIMATELY 67,000 MILES (OR LIKELY MUCH SOONER), THIS 4WD VEHICLE EXPERIENCED A LEAKING SEAL IN THE LEFT REAR DIFFERENTIAL/AXLE. THE LEAK WAS DISCOVERED BY THE DEALER DURING A RECENT VISIT TO HANDLE SEVERAL OUTSTANDING RECALL ISSUES. FROM DOING EXTENSIVE ONLINE RESEARCH, THIS APPEARS TO BE A WIDESPREAD AND COMMON PROBLEM WITH THE 4WD RAV4S. IT IS A VERY COSTLY REPAIR (AS MUCH AS $800 AT THE LOCAL TOYOTA DEALER), SO I RESPECTFULLY REQUEST THAT THE NTSB INVESTIGATE THE RAV4 REAR DIFFERENTIAL SEAL LEAKS FOR A POSSIBLE RECALL OR TECHNICAL SERVICE BULLETIN. PLEASE NOTE THAT TOYOTA ALREADY ISSUED A WARRANTY ENHANCEMENT NOTIFICATION (Z4F) WHICH EXTENDS THE NEW VEHICLE LIMITED POWERTRAIN WARRANTY FOR A REAR DIFFERENTIAL COUPLING ISSUE. I BELIEVE THAT THIS WARRANTY ENHANCEMENT SHOULD EXTEND TO THE REAR AXLE SEALS AS WELL. FAILURE OF THE REAR SEAL(S) CAN OF COURSE LEAD TO TOTAL FAILURE OF THE POWERTRAIN AND CAUSE SERIOUS INJURY OR DEATH. I HAVE ATTACHED A PHOTO SHOWING THE LEAKING LEFT REAR AXLE SEAL.

    TL* THE CONTACT OWNS A 2011 TOYOTA RAV4. WHILE IN A PARKING AREA APPLYING THE BRAKES, THE VEHICLE INDEPENDENTLY SURGED AT A HIGH RATE OF SPEED. THE RPMS RACED AND THE CONTACT'S VEHICLE CRASHED INTO TWO OTHER VEHICLES. A POLICE REPORT WAS FILED. THE DRIVER SUSTAINED WHIPLASH DUE TO THE FAILURE, WHICH REQUIRED MEDICAL ATTENTION. THE VEHICLE WAS TOWED TO A REPAIR CENTER TO REPAIR THE DAMAGES AND FURTHER INSPECT THE VEHICLE. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE. THE CONTACT WAS WAITING FOR A RESPONSE TO DETERMINE IF THEY MAY BE ABLE TO INVESTIGATE THE UNINTENDED ACCELERATION FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 76,000.

    WE RECEIVED SAFETY RECALL NOTICES NHTSA 16V-096 AND 15V-577 STATING WE WILL BE NOTIFIED WHEN A REMEDY IS AVAILABLE. I HAVE CONTACTED THE LOCAL DEALERSHIP AND WAS TOLD I WOULD BE CONTACTED WHEN THE PARTS CAME IN (THEY HAD TO BE ORDERED). I HAVE CONTACTED THE TOYOTA CUSTOMER SERVICE CENTER WHO SAID THAT WOULD CHECK INTO THE MATTER AS TOO MUCH TIME HAS ELAPSED IN THIS REPAIR PROCESS AND THEY WOULD ALSO CONTACT ME. NEITHER THE DEALER OR CORPORATE HAS FOLLOWED TROUGH WITH THE REPAIR PARTS OR REPAIRS. I STILL CANNOT GET THE RECALLED REPAIRS COMPLETED. I DON'T TRUST CORPORATE TOYOTA OR THEIR DEALERSHIP IN THEIR HANDLING OF THESE RECALLS. I DEMAND THAT NHYSA INVESTIGATE AND REPORT TO ME THEIR FINDINGS.

THE FAILURE OCCURRED WITHOUT WARNING. THE VEHICLE WAS SHIFTED INTO PARK TO STOP THE VEHICLE FROM ACCELERATING. THE VEHICLE HAD NOT BEEN DIAGNOSED OR REPAIRED. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE. THE FAILURE MILEAGE WAS APPROXIMATELY 30,000.

THE DEALER SCHEDULED AN APPOINTMENT TO INSPECT THE VEHICLE AND ORDER THE PART, BUT MENTIONED THAT THERE WAS A MASSIVE BACK ORDER FROM THE MANUFACTURER. THE MANUFACTURER WAS CONTACTED. THE CONTACT WAS NOT EXPERIENCING A FAILURE. PARTS DISTRIBUTION DISCONNECT.

THE CONTACT STATED THAT THE MANUFACTURER EXCEEDED A REASONABLE AMOUNT OF TIME FOR THE RECALL REPAIR. THE MANUFACTURER WAS MADE AWARE OF THE ISSUE. THE CONTACT HAD NOT EXPERIENCED A FAILURE.

    SINCE SUMMER OF 2014, AFTER TWO YEARS OF DRIVING MY NEW 2011 RAV4, THE DRIVER SIDE WINDOW HAS NOT OPERATED PROPERLY. SOMETIMES IT WILL GO DOWN OR UP, OFTEN, IT WILL GET STUCK. THE LIGHT ON THE BUTTON BLINKS AT TIMES, AT TIMES IT DOES NOT LIGHT UP. THE WINDOW MOTOR I AM TOLD HAS SEVERAL FINGERS THAT OPERATES THE WINDOWS AND SEEMS TO WORK WHEN TRIED ON SOME KIND OF MACHINE, HOWEVER, TWO GOOD MECHANICS CANNOT FIGURE OUT WHY IT SHORTS OUT WITH OPERATING THE DRIVER SIDE WINDOW. REPLACING THE WINDOW MOTOR IS APPROXIMATELY $460.00 FOR THE PART ALONE, BUT MECHANICS CANNOT GUARANTEE THAT THE WINDOW WILL NOT SHORT OUT SOMEHOW AGAIN. WHEN THE WINDOW IS STUCK DOWN, I DON'T FEEL SAFE DRIVING AT NIGHT, AND, WEATHER CONDITIONS LIKE RAIN, SNOW, EXTREME COLD--MAY KEEP ME FROM DRIVING THE CAR WITH THIS PROBLEM. I FOUND A RECALL NOTICE FOR THE SAME WINDOW PROBLEM; HOWEVER, TOYOTA TOLD ME THAT MY VEHICLE WOULD NOT QUALIFY BECAUSE MY VIN IS NOT IN THAT PARTICULAR RECALL. RADIO/CD PLAYER--HAS SHORTED OUT SINCE 2014. WHEN I PRESS A BUTTON TO CHANGE STATIONS, THE RADIO WILL TURN OFF. IF I PRESS REWIND ON THE CD PLAYER, THE PLAYER TURNS OFF. THIS HAS HAPPENED FOR THREE YEARS ON MY RAV4 THAT WAS BRAND NEW WHEN PURCHASED DECEMBER 2011. *TR

THE CONTACT ALSO STATED THAT THE FRONT PASSENGER SIDE AIR BAG WARNING INDICATOR ILLUMINATED WHILE A PASSENGER WAS SEATED IN THE FRONT PASSENGER SEAT. THE VEHICLE WAS NOT DIAGNOSED OR REPAIRED. THE VEHICLE WAS INCLUDED IN NHTSA CAMPAIGN NUMBER: 16V096000 (SEAT BELTS). THE MANUFACTURER WAS NOT MADE AWARE OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 54,000.

    I RECEIVED A LETTER SAYING THAT I HAD A RECALL ON THE REAR OUTWARD SEAT BELT. I CALLED ON 7/25/16 AND THEY TOLD ME THAT THEY DIDN'T HAVE A REMEDY TO THE PROBLEM. I TOLD THE REP THAT THE LETTER SOUNDED LIKE THEY HAD A SOLUTION "DEALER WILL INSTALL PLASTIC COVERS OVER THE METAL SEAT FRAME." THE REP INFORMED ME THAT ALTHOUGH A SOLUTION WAS FOUND, THEY DIDN'T HAVE THE PART. I CALLED ON 8/10/16 AND THEY TOLD ME THAT THE PARTS ARE NOT IN. I CALLED ON 10/6/16..SAME THING...PARTS ARE NOT IN. NOT SURE WHY IT TAKES 3 MONTHS TO GET THIS PART. I THINK MY CAR ALSO HAS AN ISSUE WITH THE WIPER MOTOR AND I NEVER HEARD BACK REGARDING THAT EITHER.
