The original fuel tank may have been replaced with an incorrect fuel tank, which can allow fuel to leak in a rollover crash.

    A fuel leak in the presence of an ignition source increases the risk of a vehicle fire.

The connecting rod and/or crankshaft engine components may have manufacturing defects that can lead to engine damage and engine failure.

As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 108, "Lamps, Reflective Devices, and Associated Equipment."

    Daytime Running Lights that do not deactivate as intended may cause a glare, reducing visibility and increasing the risk of a crash.

The rivet that retains the buckle to the mounting bracket in the left or right side third-row seat belt buckle assembly may have been improperly formed.

    A seat belt with a deformed rivet may not properly restrain an occupant, increasing the risk of injury during a crash.

    Dealers will inspect the rivet head formation on both the left and right side third-row seat belt buckle assemblies and replace seat belt buckle assemblies, as necessary, free of charge.     My Tahoe 2021 RST has been leaking what smells like diesel. This goes all the way back only months after I bought it new. This came and went sometimes once a week sometimes once a month. And for the next few years many months I could smell it but no leaks but in the last 10 days the diesel leak has been massive. It leaks from under the engine and if the hood is opened the diesel leak is all over the engine. The smell has been noxious sunce the beginning. Over the years the smell was there off and on. Comes and goes. But the last few months it has been everyday. Apparently the shop had fixed and replaced some parts several weeks ago but the smell of diesel never went away. Now it is in the dealer shop again. Because the fuel leak is too massive and the smell is ever worst.

    * Sudden Catastrophic Engine Failure. * The car lost power in the middle of a high traffic, very dark country road, forcing her to stay in the middle of the (undivided) road in the dark hoping cars didn't slam into her. A Collin County Sheriff was dispatched to keep her safe until the tow truck arrived almost two hours later." * The vehicle was towed to a dealer where they confirmed that the torque converter, valves, lifters had failed. Dealer didn't disclose that there was any issues for these engines, which there is. It's a 6.2L V8 Tahoe High Country. * It has only been inspected by the dealer, not the manufacturer, police, insurance representatives, etc... * The vehicle indicated a traction control problem, then the engine light came on, and then the engine failed followed by loss of power. All happened in rapid succession WITH NO OPPORTUNITY FOR THE DRIVER TO GET TO A SAFE LOCATION. * There was no warning or other symptoms preceding this. The vehicle has always been faithfully serviced at Chevy dealerships consistent with the manufacturers recommendations.

    While driving on I-90 in Montana on a clear, sunny day at the posted speed limit of 80 miles per hour for one hour, the vehicle began making knocking noises then, all of a sudden lost power. Upon inspection at the nearest GM dealership service department in Butte, MT, I was told something within the engine broke. I have attached an emailed repair bid from the GM service department and have copied and pasted the highlights here: -Engine, Lines, Radiator, Water Pump, and Tune up ($14,400.00) -Cam, Lifters, Lines, Radiator, Water Pump, and Tune Up ($11,400.00) -Master Cylinder/EBCM ($3,310.00) Previous to the incident, all recommended and routine maintenance was performed at intervals recommended by GM and by GM dealership service departments and/or the Valvoline Oil Change location in Missoula, Montana. The odometer reading was between 95,000 and 96,000 miles at the time of incident.

    I noticed the vehicle had intermittent hesitation while accelerating to merge onto a freeway on the previous evening. We drove straight home from there and the next morning as we prepared to go somewhere the vehicle lost power in our driveway and now will not start. Trouble code P129F Fault code is being thrown. This seems to be related to the GM issued TSB PIP5964 and the also GM Recall N252516900 . However, GM is denying that our VIN should be affected. I'm concerned that they're not aware of or intentionally excluding vehicles that could have this issue. Had we lost power while in motion, like on a busy freeway or crossing an intersection, this could have been catastrophic.

    I recently purchased this vehicle from a dealer in FL, but unknown to me at the time was the headlight adjuster on driver side was broken, causing the aim to drop to the bottom limit. This causes only the passenger side light to illuminate the road. I discovered this problem about a week after purchase. The dealer does not seem to be willing to correct this issue. This is a common problem for these vehicles, verified with online search's. The fix is to replace the entire headlight housing which is quite expensive, and then the labor to remove and replace the front bumper assembly.

    This vehicle has experienced multiple substantial defects, several of which directly endangered my family, including an infant, and rendered the vehicle unsafe and inoperable for extended periods despite warranty repairs. Summary of defects and failures: 1.Electronic door locking failure The vehicle experienced an electronic malfunction that caused the doors to randomly lock, resulting in my infant being locked inside the vehicle when we exited. This wiring defect was later repaired under warranty but represents a serious safety failure. 2.Fuel pump recall and highway breakdown While driving my family of six on a major highway over an hour from home, the recalled fuel pump failed while in motion, leaving us stranded on the side of the highway. We were forced to arrange an Uber for transportation. This incident placed my family in a dangerous situation and constitutes a major safety-related defect. 3.Transmission failure and extended inoperability The vehicle suffered a transmission failure that rendered it unsafe and inoperable. The dealership required over one week to schedule service and more than two additional weeks before the vehicle was inspected and repaired. The vehicle was out of safe driving condition for approximately one month. Upon return, the vehicle experienced additional failures, including a dead battery. 4.Known poor design defect – rear entertainment screens The rear entertainment system screens cracked due to a widely known poor design and placement issue, unrelated to misuse. 5.Rear headrest internal mechanism failure The internal mechanism of a rear headrest failed and requires replacement, further demonstrating ongoing quality and safety concerns. Due to the number, severity, and safety-related nature of these defects—combined with cumulative downtime exceeding 30 days—I have lost all confidence in this vehicle. I no longer consider it safe or reliable for transporting my family.

    As many have already complained, the headlight adjuster screw for the drivers side of the headlights of the vehicle always break or get stripped out even at low mileage, causing the headlight of the drivers side to point down onto the road and being unable to illuminate anything past the very front of the vehicle at night. This is a serious hazard that is not possible to fix or adjust without a large bill at the dealership. The drivers side headlight is pointed down and me myself, has resorted to adjusting the passenger side headlight to point up to compensate for this issue. This is causing many safety issues as I cannot see anything past 1-2 feet from the front.

    My Tahoe was making a loud noise, I took it to the dealer to look at it and they said I need to replace the rear driveshaft to the tune of $4000. I don't have $4000 to spend on this. This Tahoe is only 4 years old and was supposed to drive us to Minnesota next week for Christmas and now that is not possible. This is a known issue and a recall, but my VIN not included in that recall. How can this happen - you know it is a known issue in these cars!     The contact owns a 2021 Chevrolet Tahoe. The contact stated that while driving 20 MPH into the driveway, the engine seized. The contact stated that the service traction, service ESC, and engine warning lights were illuminated. Upon research, the contact associated the failure with NHTSA Action Number: EA25007 (Engine, Engine and Engine Cooling). The vehicle was towed to the local dealer, where it was diagnosed, and DTC: P0300, P050D, and P0302 were retrieved. The vehicle was not repaired. The manufacturer was notified of the issue and offered to help with the cost of the repair. The approximate failure mileage was 64,572.

    2021 Tahoe Z71 with 70000 miles making a roaring and whining noise on reverse and turning, especially when cold. Took to shop and they changed the rear differential fluid. Now its worse. Needs a whole new rear end. This veicle is 4 years old and does not tow or use four wheel drive. The shop said there has been so many complaints and services on the same issue. Why is there not a recall on this yet? We spend $70,000 and up and expect a more quality vehicle without having to spend $4000 dollars on repairs.

    I was driving on the highway at approximately 35mph when I noticed there was no response from the accelerator. I shifted the vehicle into neutral and coasted onto a side road and call OnStar for assistance. The dashboard indicator said to restart engine. I attempted to restart, but the engine would crank but not turn over (start). The vehicle was towed to the dealership where the fuel pump assembly was found to have failed and need replacement.

    Issue related to the rear-differential fluid, TSB 21-NA-028. Its a known issue where the rear differential make moan/chatter noises on turning (including at low speeds). Dealers are instructed to service/replace the rear differential fluid and perform procedures to address clutch pack chatter. It is such an issue, the fluid to remedy this is on back order!

    Based on dealer inspection, oil was found sitting on top of the #2 piston, and you’ve indicated the failure is originating from the bottom end of the engine—likely involving piston, ring, or cylinder wall damage. Combined with the stored P050D, P0300, and P0302 codes, this represents a clear internal mechanical failure. Vehicle is currently at the dealership. Mechanical failure while driving the vehicle. Sensor light for service ESC and service traction control both showed as the failure was occurring.

    The contact owns a 2021 Chevrolet Tahoe. The contact stated that while turning left with a foot on the accelerator pedal, the vehicle lost motive power and started shaking. The contact pulled over to the side of the road and attempted to restart the vehicle, but the vehicle failed to restart. The vehicle was towed to a dealer, where it was diagnosed that the intake fuel pump needed to be replaced. The vehicle was repaired, but the failure reoccurred approximately five months later. The vehicle was taken to another dealer, where the fuel pump power control module needed to be replaced. The vehicle was repaired. The manufacturer was notified of the failure. The failure mileage was 35,000.

    While driving and turning left and right, and while cold and turning It does a grinding/groan noice, and recently while driving it did that, and It started losing speed and the grinding worsening and I was losing control of the vehicle. Putting the vehicle in Auto makes the groan/grind worse.

    PER HENDRICKS CHEVROLET IN HOOVER, AL "INSPECTED REAR DIFF FOR METAL & FOUND LAARGE AMOUNTS OF METAL ON DRAIN PLUG MAGNET. RECOMMEND REPLACING REAR DIFF DUE TO LARGE AMOUNT OF METAL FOUND IN OIL." There was no warning on the vehicle; however, you can hear it loudly when a left or right turn is made.

    Driver's side headlight adjuster screw breaks very easily, and makes the headlight beam be too low with inability to adjust it. There is no way to replace it... it requires a change of the entire headlight, which requires removing the entire grill. A cheap piece that makes customers pay thousands of dollars in repairs.     I am writing to formally document serious engine defects I have experienced with my 2021 Chevrolet Tahoe equipped with the 5.3L L84 V8 engine. 1.Crankshaft Failure: My Tahoe suffered a catastrophic crankshaft failure that required major repair/replacement. I am aware that GM has recalled certain vehicles equipped with the 6.2L L87 engine for crankshaft and rod-bearing defects. Based on my experience, the same or a very similar defect has also occurred in my 5.3L engine, which is currently not included in that recall. 2.Valve Lifter Failure: In addition, my vehicle has suffered from valve lifter collapse—a well-documented problem in 2021–2022 GM trucks and SUVs with both the 5.3L and 6.2L engines. This caused severe misfiring, drivability issues, and expensive repair needs. Why this is a concern: •Both failures occurred well before what would be expected of a modern engine. •The crankshaft failure raises a safety risk: sudden loss of propulsion while driving could lead to accidents. •The lifter issue is widely reported by other owners and has been the subject of GM service bulletins. •Taken together, these problems suggest systemic defects in the 2021 Tahoe’s 5.3L engine that have not been adequately addressed by GM. Requested Action: •I ask GM to extend recall and warranty coverage to include the 5.3L L84 engine in addition to the 6.2L L87. •I ask NHTSA to consider investigating whether the scope of the current 6.2L recall should be broadened to include 5.3L engines, given failures like mine. •At minimum, I request assistance in covering repair/replacement costs for both the crankshaft and lifter defects, which are clearly not due to normal wear and tear. Answer: The vehicle’s 6.2L L87 V8 engine suffered a sudden internal mechanical failure involving the connecting rod, rod bearings, and crankshaft assembly, resulting in catastrophic engine damage. The failure mode is consistent with the defect described in GM Recall N252494001. The engine and all damaged components are fully available for inspection upon request. 2. How was your safety or the safety of others put at risk? Answer: The engine failed without warning while the vehicle was in motion, causing an immediate loss of propulsion. This created a hazardous situation where the vehicle could no longer accelerate or maintain speed in traffic, increasing the risk of a rear-end collision or loss of vehicle control. GM’s own recall documentation identifies this exact failure mode as a significant crash risk, and that same hazard occurred in my case. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Answer: Yes. A service technician confirmed catastrophic internal engine failure consistent with connecting-rod or crankshaft damage. The dealer also verified that the engine is inoperable and requires full replacement. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Answer: No, the engine failed while in motion and was taken to the closest mechanic. Inspections by an insurance representative or manufacturer representative can be arranged. The vehicle and damaged engine remain available for further examination. 5. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Answer: No. There were no warning lamps, diagnostic messages, or drivability symptoms prior to the catastrophic engine failure. The failure occurred suddenly and without any advance indication.
