Recalls and investigations

  1. Power train, manual transmissionJun 9, 2026

Ford Motor Company (Ford) is recalling certain 2015-2018 Focus and 2013-2016 Fusion vehicles previously repaired incorrectly under recall numbers 18V169 and 18V845. The clutch may fracture, resulting in damage to the transmission assembly and possibly a transmission fluid leak.

A transmission fluid leak in the presence of an ignition source such as hot engine or exhaust components can increase the risk of a fire.

Dealers will update the software, free of charge. Additionally, dealers will inspect Focus vehicles and replace the clutch assembly as necessary, free of charge. Interim letters, notifying owners of the safety risk, were mailed August 5, 2026. Additional letters will be sent once the remedy is available. The rearview camera may display a distorted, intermittent, or blank image when the vehicle is in reverse.

A rearview camera that fails to properly display an image can reduce the driver’s view behind the vehicle, increasing the risk of a crash.

Dealers will inspect and replace the rearview camera as necessary, free of charge. This will be a phased campaign, with the remedy becoming available in different phases based on model and model years. Interim letters notifying owners of the safety risk were mailed on October 25, 2025. Additional letters will be mailed once the final remedy is available on each phase. A damaged or missing bushing may prevent the vehicle from shifting into the intended gear, and cause the vehicle to move in an unexpected direction. Additionally, the vehicle may roll after the driver selects the 'Park' position. Either scenario increases the risk of a crash or injury.

Dealers will replace the under hood shift bushing and add a protective cap over the shift cable bushing, free of charge. Interim owner notification letters, informing owners of the safety risk, were mailed July 1, 2022. Remedy parts are expected to be available in 4th Quarter 2022. Ford Motor Company (Ford) is recalling certain 2014-2015 Fiesta, 2014-2016 Fusion and Lincoln MKZ vehicles sold, or ever registered in Alabama, Arkansas, Arizona, California, Florida, Georgia, Hawaii, Louisiana, Mississippi, New Mexico, Nevada, Oklahoma, Oregon, South Carolina, Texas, Utah, Washington, Puerto Rico, American Samoa, Guam, the Northern Mariana Islands (Saipan), and the U.S. Virgin Islands. A component within the door latches may break making the doors difficult to latch and/or leading the driver or a passenger to believe a door is securely closed when, in fact, it is not.

A door that is not securely latched could open while the vehicle is in motion, increasing the risk of injury to a vehicle occupant.

Ford will notify owners, and Ford or Lincoln dealers will replace the side door latches, free of charge. The recall began September 21, 2020. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 20S15.

My antifreeze AND oil keep going empty after driving the car after so many days..... there is no leak on the ground, and i do not smell anything inside the cab. No lights are on the dash I took it and has them plug it in just incase and the only thing that came back as a possible issue is the brake pad. Otherwise they said there was no issue However, there is an issue and no one can seem to figure out what it is!

  3. Electrical system, Other or unspecified, Visibility/wiper, Sep 23, 2026

The backup camera is malfunctioning. I brought in the car to the McGovern ford dealership for repair, but they stated the remedy was not yet available. It’s been over a year with a distorted image displayed on the screen, and it causes a huge distraction/confusion while reversing. It occasionally goes black, causing a dangerous situation while reversing in public areas. It began displaying a “repair needed, contact ford dealership” notification only after the camera had been distorted. This puts pedestrians/other vehicles at risk because it inaccurately displays where they are/ how close in proximity they are to my reversing vehicle. I have to turn my head to check my surroundings, but the distorted image displayed on the screen distracts/confuses the driver. It has been confirmed by the ford dealership.

  8. Other or unspecified, Lane departure, Back over prevention, Sep 18, 2026, crash

The component that fail was the backup camera, and the dashboard screen was black. I was looking at the screen backing up, and I seen that the camera was black and had like colorful lines on it, and also my screen. One side was like a black spot inside of their screen on the deck of the radio. I proceeded to back up and turn my wheel and that’s when I a bump and grinding sound. Therefore, once I stopped backing up, I got out the car in my passenger rear bumper was hanging down, and I had a dent & white paint is coming up the side of the back passenger rear end. The paint that came on the rear passenger side was from me begging into the pole. I have called several times teo different Ford dealerships to see if they have the part for my car the camera & dash screen. Unfortunately, I was informed that the part have not came in and then I need to wait until another notice but I seem to have an accident since I called in April of this year 2026, and I also called in May and now I have had accident in June from this problem. Ford should have the parts or replace with updated parts. Due to this I am scared of backing into something else. I hope Ford can fix this recall on my Ford Fusion. Instead of getting legal advice for this issue. I just would like for this recall be resolved.

The contact owns a 2016 Ford Fusion. The contact received notification of NHTSA Campaign Number: 25V695000 (Back Over Prevention). The contact stated that while reversing in the driveway, the back over prevention camera became inoperable. The message “Camera Unavailable” was displayed. The Ford call center was contacted by phone and advised the contact that the camera might not be repaired until the remedy was available.

  10. Engine, Sep 17, 2026

Exhibited signs of a coolant leak followed by throwing a code about cylinder 4 misfiring. Took my car to the dealer who confirmed that there is coolant leaking into cylinder 4, and it is similar to other defects found in many 1.5 liter ecoboost engines from this model line. Was told that my VIN did not qualify for the recall that is only for the ecoboost 2.0 liter engines even though they have the same defect.

  11. Other or unspecified, Sep 17, 2026

I own a 2016 Ford Fusion that is included in NHTSA Recall Campaign 25V695 and Ford Recall Campaign 25SA9 concerning rearview/backup camera failure. The backup camera intermittently fails to display video when the vehicle is shifted into Reverse. Instead of showing the view behind the vehicle, the display shows an error message. This problem has been occurring intermittently for the past few years. I received notification of the recall inXXX. In XXX I contacted Loveland Ford in [XXX], regarding the recall repair and was told that Ford expected parts to be available in XXX. On SXXX, I again contacted Loveland Ford and was told that they did not have any timeframe for when parts would be available. The most recent backup-camera failure occurred on XXX (pictures attached). When the camera fails, the driver loses the rearward video view while backing and must rely only on mirrors and direct visual checks. Because the failure is intermittent, the camera cannot be relied upon to operate when needed. My [XXX] son began driving this vehicle in XXX. Reliable operation of the backup camera is especially important for a newer driver, and the continuing lack of a repair or estimated parts availability leaves an unresolved safety concern. I am submitting this complaint to document the recurring backup-camera failure and the continued unavailability of a recall remedy for my vehicle. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

  12. Electrical system, Back over prevention, Sep 15, 2026

    1. System / Component Failed: The rearview camera system displays a blank, distorted, or black screen while the vehicle is in reverse, completely eliminating rearward visibility. 2. Safety Risk: The loss of backup camera functionality directly impairs critical rearward visibility while reversing, significantly increasing the risk of a backover crash, striking pedestrians, or colliding with obstacles. 3. Manufacturer & Dealer Confirmation: This vehicle is affected by Ford Safety Recall Campaign 25SA9 (NHTSA Campaign 25V695). Local Ford dealership service departments and Ford Corporate have been contacted, and both confirm that no remedy or repair parts are available to perform the recall service. 4. Failure Details & Unreasonable Recall Delay: The safety defect is active and reproducible every time the vehicle is placed in reverse. Although this recall was issued nearly one full year ago (October 2025), the manufacturer has failed to supply replacement parts or provide a timely remedy under federal recall standards. The safety feature remains entirely inoperative without a resolution date or estimate provided by the manufacturer.

  13. Power train, Sep 13, 2026

The flexplate has failed twice since a dealer-performed transmission replacement in June 2024, once in August 2025 and again in August 2026. A flexplate failure causes sudden loss of the ability to move the vehicle under power, which is a safety hazard, particularly while merging, accelerating onto a highway, or in traffic. A Ford service bulletin covering the closely related Escape with the same engine and transaxle architecture documents that flexplate cracking is linked to engine-to-transaxle alignment issues that arise whenever the engine and transmission are separated, which is exactly the service performed during a transmission replacement. This report is filed because the recurrence of an identical failure mode, on a known-affected platform, without resolution by the servicing dealer, appears to reflect an unresolved and potentially widespread mechanical defect rather than an isolated incident."

  14. Back over prevention, Sep 11, 2026

I was notified in October 2025 by Ford of a recall (NHTSA Recall 25V695) for my 2016 Ford Fusion's rear-view camera. We have contacted Ford numerous times as we have not received an additional letter stating to take the vehicle in for repair. They are claiming to only have the rear-view camera replacements for the Mustang. The rear-view camera on my vehicle does not work when in reverse (black screen that says to take the vehicle to the dealership for repair). This is a safety hazard and has almost been 1 year since the initial notification.

  20. Electrical system, Visibility/wiper, Sep 1, 2026

I am fully aware that my vehicle has a recall for the backup camera. Manufacturer recall number 25SA9, NHTSA recall number 25v695. That was back on XXX. Almost a year ago. It would be super if I could use my backup camera. I really appreciate that feature in the store parking lot, backing out of my driveway on a busy road, and picking up and dropping off my child from school as there are kids and vehicles everywhere are a few examples. Yes, I look over my shoulder regardless, but that camera does help a lot especially around the little kids. I have had a few close calls because someone was directly behind me and I didn't see them. Had my backup camera been working like it should have that wouldn't happen. How has it been almost a year and they still don't have parts? I have already had a few other recalls on my car to deal with as it is, but this one is particularly frustrating. How much time is a manufacturer typically given to fix a problem like this? How long do I need to wait for a feature that I paid for to work? I am a little frustrated, I know it is not your fault at all. I am just looking for some kind of an answer here. Thank you, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
